Today is World Patient Safety Day. This year's theme: safe care for non-communicable diseases. NCDs are now projected to cause nearly half of all deaths in Sub-Saharan Africa by 2030. Behind that number are people who will spend the rest of their lives attending clinics: monthly refills, repeat checks, follow-ups that have to carry information from one visit to the next. Risi is one of them. She's 51, living with hypertension for 13 years at General Hospital, Orile-Agege, in Lagos, Nigeria. For years, her records weren't captured in a way the next clinician could use. An assessment against SafeCare standards found the familiar gaps: documentation, medication management, infection prevention, no reliable way to track patients seen again and again over years. What followed was a Quality Improvement Plan with named owners and real deadlines, not a report that sat in a drawer. A facility QI facilitator, trained through the state's quality improvement programme, now supports colleagues to do the same. In June 2025, the facility was named Most Active Public Facility on the SafeCare Quality Platform at the Nigeria Health Excellence Awards. Risi's record can now be retrieved. Her readings are trended, not lost. The clinician who sees her in December can see what was documented in March. Every handover is a point where harm can enter, and a point where it can be designed out. That is what safe care looks like. Not heroic.
