Cannot add a connection due to error: command 'sqltools.newSqlFile' not found

To Reproduce
Click on Add New Connection
VSCode error popup: Error running command sqltools.newSqlFile: command 'sqltools.newSqlFile' not found. This is likely caused by the extension that contributes sqltools.newSqlFile

Expected behavior
New connection dialog opens

I haven't been able to reproduce this.

Is it the blue "Add New Connection" button you're clicking? or the one on the titlebar?

Does it also happen for you if you wait a few seconds before clicking the button?
