Dr. Raunaq Pradhan posted thisAnd what a satisfying last 10 days it has beeeennn! 🌸 💜 It started with a simple trigger — a post from GrowthX® about doing an AI weekender. Thank you UD! I signed up partly out of curiosity, partly out of a feeling I hadn’t fully kept up with the shift towards becoming an “AI PM”, and maybe a bit of guilt from skipping a previous buildathon to prioritise my health. What followed has been some of the most fun I’ve had in a long time. 1) I found myself obsessing over building again — to the point where I exhausted my limits on Claude Code (more than once 😅). That feeling of solving something end-to-end is hard to replicate. 2) After speaking to a few founders and drawing from past frustration, I started working on a problem that kept coming up: -> How do you simplify healthcare regulations and clinical documentation for founders? -> Built a quick prototype → got real interest → and now exploring it further: https://lnkd.in/gnKvheJA 3)Took it a step further for the IndiaAI CDSCO Hackathon — building from the reviewer’s perspective this time. Instead of just helping applicants, how do we simplify document-heavy regulatory workflows for the system itself? Submitted the proposal, and learnt so much in the process! 4) One big takeaway: -> It’s very easy to overbuild with AI. -> Clarity still comes from scoping, simplifying, and sometimes just going back to pen and paper. -> GTM is as or rather more important, than the product itself! 5) Also revisiting something I deeply enjoy — thinking like a researcher again with Paras Chopra. Currently exploring how biomedical statements hold truth across different LLM framings (Lossfunk). Curious where this goes. And somewhere in between — good food, proper sleep, and a weekend of just slowing down. 💛😇 🙌 Grateful for the energy, the people, and the process. 🙌 🖐 💝 Would love to connect with folks building at the intersection of AI × healthcare × public systems. All assembled at the Indian School of Business for something very consequential to India right now. Artificial Intelligence in Public Health. The hard part about AI in public health is not the technology. It is getting the right people in the same room at the same time. Epidemiologists who understand disease burden, clinicians who understand care delivery, administrators who control systems, implementers who have actually rolled out digital health on the ground, and policymakers who can move the needle at scale. That room is genuinely hard to assemble. ISB Hyderabad assembled it. Given the rarity of the opportunity, I did not want to engage this room in plain vanilla AI in health narratives. I wanted to tell the real India story — the story of how our AI journey did not start in 2020. It started decades earlier, with telemedicine pilots, hospital MIS, CoWIN, Aarogya Setu, and eventually ABDM. All that hard work building India's Digital Public Infrastructure is now the foundation on which AI is riding. And India is no longer thinking about this in concept. States are already using AI to predict Dengue outbreaks. Tools are being deployed in rural vision centres, TB screening programmes, and community health workflows. For a nation of 1.4 billion people with constrained health resources, effective AI in public health will not just improve outcomes, it will meaningfully change India's trajectory. India has one hell of a story in digital health and AI. The session brought together policymakers, researchers, implementers, and ecosystem leaders to explore pathways for building interoperable and actionable climate-health data systems grounded in public purpose. Through keynote reflections, implementation use cases, and strategic dialogue, participants examined critical gaps in data availability, governance, and institutional coordination, while identifying how Digital Public Infrastructure (DPI) and responsible AI can help integrate climate, health, and social datasets to support evidence-driven decision-making. The roundtable also spotlighted emerging efforts toward a Climate–Health Data Collaborative, emphasising the importance of cross-sector partnerships, ethical data stewardship, and sustainable models that move beyond pilots toward scalable ecosystem solutions. He is working with Dharkan Anand from ClimMaTech, predicting floods and extreme climatic events, and Gordon Rates from AirNode specializing in air quality analysis. The team is building an AI-driven model for preparing public health departments to combat health and climate risks using early warning systems. The framework adopted analyzes data on flooding, vector borne diseases and air quality index to create a climate and health risk platform that warns and prepares different stakeholders of the public health system. Raunaq’s team is exploring launch of this combined predictive model in Delhi as a pilot program with the ultimate goal of extending this to the UK for better preparedness for controlling vector borne disease transmission and other climatic disasters in India and in the UK. This one had a different pulse: a real emphasis on digging deep into problems, surrounded by genuine experts - doctors and entrepreneurs like Dr. Pranav Agrawal - who jumped in with spot-on feedback, guiding us every step from raw idea to polished prototype. Our team (Lokesh Bansal and I) zeroed in on the chaos of unpredictable appointments during OPD visits for patients. We crafted a patient-side app that tackled those headaches head-on , while layering in smart intelligence to optimize scheduling (priority, grouping, routing irrelevant appointments) I walked away buzzing with energy and optimism about India's healthcare scene - there's so much innovative magic brewing here! Dr. Raunaq Pradhan liked thisDr. Raunaq Pradhan liked thisBefore quick commerce became India’s favourite business model, I had once attempted to make 1-hour blood delivery work. This was 2014. Found this screenshot from my first startup, and it took me right back. At 17, what started as an engineering project slowly became my first real lesson in building for messy systems. Back then, in many smaller hospitals, getting access to blood was still painfully manual. Families often had to depend on doctor references, call larger hospitals or blood banks, coordinate availability, and then arrange a replacement donation. On the surface, it was a blood bank management system. But the problem we identified was far beyond “where is blood available?” It was trust. So even in that very early version, we were thinking about verified institutions, donors linked to approved blood banks, digitising health markers for every patient and donor, and an admin layer that could help govern the network. At 17, I didn’t have fancy words like “healthcare infrastructure” or “supply-demand dynamics”. I just knew this could not be another listing platform. I reopened the idea during my MBA, worked with KEM Hospital to create awareness, and received an iCreate Accelerator grant through the Department of Science & Technology, India. For a while, it felt like the project had a second life. However, life moved, and I couldn’t pursue it full-time. And I learnt very early that building a product and building a business are not the same thing. Eventually, we shut it down. This screenshot is from that Java project. No sleek demo. No founder brand. No pitch deck aesthetics. Just a 17-year-old trying to solve a real problem, with a lot of curiosity and a very questionable UI. Looking back, I realise I have always been drawn to the same kinds of problems: messy systems, fragmented workflows, trust gaps, too many actors, too much manual coordination. There’s odd comfort in that. I’m building again now, and on days when the ambition feels slightly delusional, or the path feels impossibly hard, it’s oddly comforting to remember this version of me existed too.

Onions, salt, biscuits, toothpaste, Harpic: all those tiny things for which we now routinely summon a human being, a bike, a plastic bag, a dark store, venture capital, imported crude, and the full might of 10-minute delivery startups. Brain used. Oil saved. Patriotism delivered. Win-win-win.

Dr. Raunaq Pradhan liked thisDr. Raunaq Pradhan liked thisImagine a clinical trial designed for 100 people. Now imagine only 1 of them looks anything like you. That's roughly where South Asia stands in the world of modern genetic research today. We are a quarter of the planet's population — and less than a fraction of a percent of the people whose DNA has shaped what doctors know about disease, drug response, and what "normal" even means. So when your doctor prescribes a medication based on global research, there's a good chance it was never really studied on someone like you. That's the data gap we're trying to close. And it's the reason I'm proud to share that our paper is out today in The Lancet Regional Health – Southeast Asia. "The case for an integrated biobanking initiative in South Asia" lays out a ten-year roadmap to get nearly two billion people out of biomedicine's blind spot. Linked health data resources across India, Pakistan, Bangladesh, Nepal, and Sri Lanka — coordinated regionally, but built so that no country's health data ever has to leave its own soil. Working on this paper crystallized something for me about the two halves of my professional life: 1. At the Indian Council of Medical Research, the question has always been: how do we build the evidence so that the medicine of the future actually works for an Indian patient? 2. At Medtronic LABS, the question is: how do we build the delivery — the community health workers, the technology, the unglamorous last mile — so the science we generate actually reaches a patient sitting in a village clinic? The paper argues, in writing, that a project this big won't work without community health workers leading recruitment, consent designed for the languages and literacy levels of the people involved, and a fierce defense against the long history of South Asian data being mined for someone else's benefit. That's not a footnote. That's the whole game. Evidence without delivery is a paper nobody reads. Delivery without evidence is well-intentioned guesswork. South Asia needs both — built by us, for us. Because the second one means the first one actually worked. I've been thinking about why so many AI implementations don't get a second chapter. It's because nothing was built to last after the excitement faded. Here's what I've noticed across every client we've worked with at ALTRD — Every company has one person who just gets it faster than everyone else. >> The marketer who started using AI to cut her brief-writing time in half. >> The analyst who built three automations nobody asked him to build. >> The ops lead who figured out a workflow shortcut and started sharing it in the team WhatsApp. That person is the most valuable AI asset in the organisation. And almost every company treats them like a happy accident. They become the unofficial AI person. Everyone goes to them. They answer the same questions forty times. They start carrying the entire momentum of the company's AI adoption on one set of shoulders. And then they get promoted, or they burn out, or they leave — and everything they knew walks out with them. We've seen this happen. More than once. 1/ So the first thing we do in any sustained engagement is find that person. We call them AI champions. Give them a mandate, protected time, and a structure that turns what they know into something the organisation owns. 2/ AI Bytes : Five minutes. Every day. One AI development, one tool, one industry news - connected to work the team is actually doing that week. Because habits outlast enthusiasm every single time. We built this into our own team at ALTRD before we ever brought it to a client. The teams that stick with it show meaningfully higher adoption six months out than the ones that don't. 3/ The third thing is measurement. Every quarter we go back to the baseline. Where was this team when we started. Where are they now. What moved. What didn't. What's the next workflow worth redesigning. When a CXO sees their organisation has genuinely moved - not on a consultant's word, but on their own data - they don't need a sales pitch for the next engagement. They see the trajectory and they want to keep it going. Just work that proves itself. Systems that run after we're gone. And a team that's measurably different from the one we walked into. The second engagement takes care of itself.

India’s digital health story is shifting from hopeful startups to structural scaffolding, and that matters more than shiny apps. Signals of change already in motion: → ABDM / NDHM (health IDs, consent frameworks) are creating national rails for identity and basic data portability. → Telemedicine regulation updates are legitimizing hybrid models of care and clarifying provider liabilities. → Public–private partnerships are piloting real-world integrations (diagnostics - primary care → tertiary referrals) in aspirational districts. But infrastructure is plumbing, it’s boring, slow, and essential. Most founders still build as if the plumbing doesn’t exist. What will win in the next 2–3 years: → Interoperability-first products that assume ABDM rails are table stakes and design for consent, verification, and low-bandwidth contexts. → B2B2C models where hospitals, employers, insurers, and government programs are the distribution partners, not mere endpoints. → Data-infrastructure plays: consent managers, lineage/audit logs, anonymization & synthetic data services, governance tools, these will be high-leverage. → Localized usability: products that solve non-English UX, offline sync, and local clinical workflows will outcompete global-first UX-only apps. → Evidence-first pilots that link digital interventions to claims or administrative datasets to demonstrate real fiscal impact. One of the core problems in Indian healthcare is the very high out-of-pocket expenditure. And while from 2013-14 to 2019-20 (the stabilised post-COVID data is not yet available), it has fallen by 17% from 64% to 47% of total health expenditure (worryingly, almost 12% of that was due to a fall in total health expenditure itself and not increased government spending), it still remains quite high. One potential pathway that has been much discussed is government-financed insurance (earlier known as RSBY and now as PMJAY), which, at Rs. 10,000 crore annually, currently accounts for about 0.03% of GDP or 1.5% of government health expenditure. This is currently spread over a 60 crore population, i.e., about 170 per capita, against a total health expenditure of about Rs. 7,000 per capita. There are a number of studies (https://lnkd.in/g-eVB4Ut) that have focused on analysing the impact of this Rs. 170 and the role it can play in UHC. It is important to review all government programs and explore opportunities for improvement, not just at the implementation level but also at the design level. In the context of insurance (including PMJAY), I would submit that there is a critical macroeconomic hazard. This is the fact that it operates on a reimbursement basis for each episode of illness/treatment, either on a fee-for-service (FFS) or diagnosis-related group (DRG) basis. If this design-induced hazard is not addressed, as the US discovered, improved funding and implementation could make the situation far worse. As Pauly (1968) established in his seminal critique of Arrow’s (1963) foundational work, providing third-party insurance that lowers the consumer's marginal cost of care inevitably leads to rational economic overutilization. When paired with an FFS/DRG payment structure, this generates increased demand (and supply), financially incentivising the overprescription of lucrative hospital care (Bau, Das, & Chang, 2023). The historical precedent for this is stark. The 1965 implementation of Medicare in the U.S. injected massive federal funding into a similar FFS/DRG and retrospective cost-based framework (Mayes & Berenson, 2006). As Finkelstein (2007) demonstrated, this macroeconomic shock caused a 37% increase in hospital expenditures within five years, locking the U.S. into an irreversible trajectory of skyrocketing costs and suboptimal, volume-driven outcomes characterised by systemic overtreatment (Berwick & Hackbarth, 2012). Indian insurance has largely bypassed the structural health economic reforms of the 1990s and 2000s—namely, managed care, capitation, and regulated competition (Enthoven, 1993; Porter & Teisberg, 2006). Pouring capital into a 1960s US-style system that rewards the volume of sickness rather than the value of health will inevitably yield systemic waste. But as a founder working on the ground, especially across Tier 2 and Tier 3 cities, I’ve realized something crucial — The pace of innovation must match the adaptability of the people it serves. The Promise is Clear Healthcare in India is evolving fast — and the shift is undeniable: - Digital-first care: Border less consultations through national platforms. - Preventive focus: Home testing and diagnostics redefining early detection. - Tech infusion: AI, IoMT, and data analytics enabling smarter diagnosis. - Digital Therapeutics (DTx): Clinically proven software transforming chronic care like diabetes. - Insurance expansion: Ayushman Bharat widening access for millions. - Tier 2 & 3 growth: Private investments bridging the urban–rural gap. ⚠️ The Ground Reality While innovation scales fast, access and trust must scale faster. Resilient Healthcare Financing: Lessons from Pandemic Recovery Models The pandemic taught the world a lesson no balance sheet could predict — that resilience in healthcare is not built overnight. When COVID-19 disrupted economies, it also exposed the vulnerabilities in how healthcare systems were financed, managed, and sustained. Out of that crisis emerged a new model of thinking: resilient healthcare financing. In the post-pandemic era, both governments and private players have been rethinking how hospitals and health systems are funded. Instead of short-term project funding, the focus has shifted to long-term sustainability and preparedness. Many countries, including India, introduced blended financing mechanisms that combined public funding, private investment, and development aid to build stronger, more adaptable systems. Governments launched credit guarantee schemes, viability gap funding, and emergency liquidity facilities to keep healthcare institutions solvent during turbulent times. These measures didn’t just plug financial holes — they laid the foundation for more shock-resistant capital structures. Private healthcare providers, on the other hand, embraced financial agility — diversifying revenue streams, digitizing operations, and adopting asset-light expansion models. The integration of telemedicine, home healthcare, and insurance-linked payment systems helped organizations stay financially stable even when patient footfalls declined. Another critical shift was the rise of impact and ESG-based investments. Investors began valuing not just profitability but also resilience — the ability of healthcare institutions to serve communities during crises. Hospitals with transparent governance, strong supply chains, and digital readiness became the preferred destinations for long-term capital. Resilient healthcare financing, therefore, is not about reacting to emergencies; it’s about designing systems that can withstand them. It’s about ensuring that when the next disruption arrives, our hospitals, diagnostic centers, and healthcare workers are financially equipped to continue serving without compromise. The future of healthcare depends not only on innovation in medicine but also on innovation in financing.
