Illumination Health + Home is a nonprofit 501(c)(3) organization that provides targeted, interdisciplinary services for the most vulnerable homeless adults and children to disrupt the cycle of homelessness. The organization provides a full continuum of care from street to home, including case management, medical coordination, behavioral health and substance use counseling, and housing navigation to our vulnerable clients in shelters/navigation centers, family emergency homes, recuperative care/medical respite facilities, and permanent supportive housing.

Illumination Health + Home had the opportunity to participate in the Leading for Impact program led by The Bridgespan Group in partnership with The Samueli Foundation. Through this experience, our team engaged in intensive workshops focused on strategy, organizational effectiveness, and team leadership. Through this experience, our team strengthened strategic leadership skills, sharpened decision-making, and deepened alignment around our mission. We’re excited to bring these insights back to our work as we continue serving individuals and families experiencing homelessness across Orange County. The 2026 Illumination Health + Home’s Chef’s Table Gala raised more than $1.75 million for unhoused families in Orange County. This year’s gala also recognized two outstanding partners whose work reflects Illumination’s mission to break the cycle of homelessness. One of those partners was the Bera Family, honored for their multigenerational commitment to education, healthcare access and community advancement. “It was a wonderful night,” says Rimal Bera, MD, a professor of psychiatry & human behavior at UC Irvine and clinician with UCI Health. Bera enjoyed celebrating alongside Congressmembers Dave Min, Derek Tran, Lou Correa, and Ami Bera (his brother).
