Telehealth keeps Community Health Center patients connected to care, even when life makes it hard to walk through the door. Elizabeth Mastrianni, Nurse Practitioner at @UrbanHealthPlan, a community health center network serving the Bronx, Harlem, and Queens, sees it every day. A parent juggling multiple kids. Patients working two or three jobs. People who would otherwise delay or skip care altogether. Telehealth changes that. It brings care into the home, gives patients their time back, and opens the door to more complete, proactive care. But right now, New York reimburses health centers for telehealth at only a fraction of in-person rates. That limits how many patients they can reach. We’ve seen what works. Now we need policy that keeps up. Don’t shortchange telehealth! Support full payment for CHCs! Watch the video to hear what’s possible. #ValueCHCs

Patients don’t just need care. They need someone on their side. For Sandy, that someone is a doctor she trusts with her life, backed by a team that listens and cares. That’s what New York’s 2.5 million community health center patients know they can count on. Can they count on Albany to invest $300 million to protect that care? #ValueCHCs

When Kiki’s son was seriously injured, everything changed in an instant. In the middle of fear and uncertainty, their family's community health center was there. From occupational therapy to dental care, their health center helped support Kiki’s son's recovery and gave her family a place to turn when they needed it most. For 2.5 million families like hers, community health centers like Neighborhood are more than a provider. They are a steady source of care, compassion, and support when life takes an unexpected turn. New York should invest $300 million in community health centers so more families can count on that care, too. #ValueCHCs

Ann Marie knows what it means to count on a community health center. She counted on her health center as a child. Today, her children, grandchildren, friends, and family count on it too. She believes in it so deeply that now, she works there. Her story is personal, but it’s also shared by more than 2.5 million New Yorkers who count on community health centers for care close to home. Albany must do more. Invest $300 million in community health centers.
