Beyond the clinical charts and daily schedules, there is a human element to nursing that changes lives. Our AVP of Nursing, Karelle Webb, MPH, RN, NEA-BC, recently shared her perspective on the vital role our nurses play at Oak Street Health. It goes far beyond clinical support; it’s about the deep advocacy and education that gives our patients the confidence to manage their health. To our entire nursing team: Thank you for being the dedicated bridge between our care plans and our patients' success. :green_heart: #NursesWeek #Nurses #NurseAppreciation

As John Schumann, MD explains, joint pain is often a biomechanical issue: when muscles weaken, joints bear the load. Simple movements like chair squats help "oil" the joints and restore mobility. It’s a reminder that the most effective "treatment" is often a clear explanation. This is value-based care in practice, ensuring patients leave with more than just a diagnosis. Your Medicare Annual Wellness Visit is more than a routine check-in. It’s a chance to step back, look at the full picture of your health, and plan for what comes next. Too often, patients leave these visits wishing they had asked more or different questions. A little preparation can turn that visit into a powerful, personalized conversation about your health, your risks, and your goals. At Oak Street Health, part of CVS Health, we focus on delivering personalized, preventive care designed around the unique needs of older adults—starting with meaningful conversations like these. We put together 5 simple but important questions to help guide that conversation, from medications and screenings to staying independent and planning follow-up care. These aren’t just clinical checkboxes. They’re the building blocks of better outcomes, stronger relationships with your provider, and a more proactive approach to aging well. Taking the lead in your health starts with asking the right questions.

Grateful to our very own Antonio Sanchez Gonzalez, MSN, MHA, APRN, FNP-C, for sharing his perspective on team‑based primary care in NEJM’s Not Otherwise Specified podcast. This episode explores a big question in health care right now: if the future of primary care is team-based, what actually makes a team work? Antonio shares his perspective about the need for more standardized, accredited training programs for nurse practitioners and PAs as their role in health care continues to expand. He explains that the future of primary care is collaborative with APPs and physicians working together to deliver high-quality care, and I couldn't agree with him more. The Oak Street Health New Graduate NP Fellowship program, which received accreditation last November, allows fellows to spend time building their own patient panel, learning from a multitude of preceptors, and engaging in advanced content that aims to build on graduate training. To learn more about our clinical fellowships, click here: https://lnkd.in/dRVhGWdA. And thank you, Antonio, for bringing practical experience into the conversation.

In this video, #OakStreetHealth provider Rene Roberts, MD, MS, FAAFP explains why chronic inflammation is a key risk factor for older adults and how they can manage it. It's an example of how we give our providers the time to identify risks early and teach patients how sleep, stress, and nutrition affect long-term health. Our value-based model allows you to move beyond quick fixes to focus on prevention. If you want to practice medicine where primary care is a specialized field, we want to hear from you.
