Role Overview
The Clinical Success Navigator (CSN) is a hybrid onboarding and clinical coaching role responsible for guiding clients through their full journey, from first referral to ongoing coaching success.
You will serve as the first point of contact, the clinical onboarding lead, the primary coach and accountability partner, and the bridge between clients, providers, and the RenewRx care team.
Key Responsibilities
1. Client Onboarding & Activation
- Conduct 30–45 minute virtual Welcome & Onboarding Calls
- Introduce clients to RenewRx, our platform, and care model
- Guide clients through app setup, CGM integration, glucose tracking, and food diary setup
- Troubleshoot technical or onboarding challenges
- Schedule and transition clients into ongoing coaching sessions
- Document onboarding details and send follow-up communication
2. Clinical Coaching & Ongoing Support
- Lead one-on-one virtual coaching sessions
- Support pregnant women managing diabetes and or hypertension in pregnancy
- Utilize motivational interviewing and behavior change techniques
- Create easy to follow intervention plans using tracked health data
- Provide accountability and monitor progress
- Collaborate with providers for coordinated care
3. Care Coordination & Collaboration
- Collaborate with internal clinical leadership
- Participate in team meetings and case reviews
- Ensure adherence to clinical guidelines and documentation standards
4. Client Experience & Education
- Create a positive, inclusive coaching environment
- Develop or contribute to educational content
- Serve as client liaison for questions and troubleshooting
Performance Metrics
- Successful onboarding and activation rates
- Client engagement and attendance
- Client satisfaction and health outcomes
- Documentation accuracy and timeliness
Qualifications
- Board certified Nutritionist (or equivalent credential), behavior change focus an asset
- 1–3+ years experience in nutrition coaching or healthcare
- Experience in diabetes, prenatal, or chronic care management
- Comfort with virtual platforms and CRM systems
Preferred
- Spanish-speaking
- Experience with glucose interpretation
- Training in motivational interviewing
