Basic Function
Position is responsible for providing comprehensive nursing services to clients and their families including assessment
and education in coordination with team-based care. Performs care management of patients with complex conditions
within the primary care and family practice setting.

Basic Function

Iii. Primary Responsibilities
- Participates as an integral part of a multi-disciplinary team for the comprehensive health care of clients and their

- Provides education about disease processes and management (e.g. diabetes, asthma, and COPD).
- Consults with members of multidisciplinary care team (Community Health, Nutrition, Behavioral

- Works with providers to support team efforts around care coordination and the model for improving metrics

- WORKING RELATIONSHIPS:

Vi. Skills And Abilities
- Excellent clinical skills.
- Experience with EMR; intermediate proficiency with Outlook and MS Office applications, such as Word and

- Excellent customer service and teamwork skills.
- Ability to always maintain a professional attitude and demeanor.
- Graduate of an accredited school of nursing required; BSN preferred.
- Licensure as a Registered Nurse in the State of Missouri required, and in good standing.
- Two plus (2+) years of experience in nursing working with patients who have complex conditions requiring care
- Experience working with patients in a primary care setting. OB experience preferred.
