#34Lives is proud to join other leaders across the US in a new collaborative initiative, the Kidney Rescue and Perfusion Alliance (#KiRPA). This coordinated, multi-center alliance will focus on advancing kidney rescue, perfusion science and clinical application. We believe that this group can accelerate progress in advancing normothermic machine perfusion (NMP) for kidney transplantation in a way that is not possible with a less coordinated and collaborative effort. The Alliance will be organized around two teams: Basic Science and Translational Research and Clinical Research. Dr. Jacqueline Garonzik Wang, who is leading the initiative, states, “This is an exciting moment for kidney perfusion science. I am pleased to share our latest research published in the Annals of Interventional Pulmonology, where we explore the role of Transbronchial Lung Cryobiopsy (TBLC) as a strategic "diagnostic bridge" for patients on Veno-venous (V-V) ECMO. In the critical care setting, we often face a diagnostic dilemma: a patient with refractory ARDS requires a histological diagnosis to guide treatment, but the risks of surgical lung biopsy are prohibitively high. Our study demonstrates that TBLC offers a feasible, less invasive alternative with high diagnostic yield. Key Highlights: Clinical Impact: In our index case, TBLC successfully established a diagnosis of organizing-phase Diffuse Alveolar Damage (DAD). Targeted Therapy: This histological clarity allowed us to transition from empirical treatment to targeted pulse steroid therapy, leading to successful ECMO decannulation and patient recovery. Safety Profile: Our systematic review of 30 cases across world literature found no procedure-related mortality, supporting the safety of this bedside intervention when performed by an experienced team. This work reinforces that ECMO does not just sustain oxygenation—it provides the physiological stability needed to perform critical diagnostic maneuvers safely.
