Letter of Recommendation Form
If you apply using the UW System Application, your recommender can use our online recommendation form. This system allows you to request letters from each of your chosen contacts. By creating a login and entering your information, your recommender will receive an email with a link to upload a letter to our office. Those who apply using the Common Application should request a recommendation through that system.
Application Fee Waiver Request (PDF)
The application fee may be waived in cases of exceptional need. Eligible students can request a fee waiver as part of your application. If you did not request an application fee waiver at the time of application, but are eligible to have your fee waived, you may complete the Application Fee Waiver Request Form and email it to [email protected].
Enrollment Deposit Form (PDF)
In order to secure your place in the upcoming class, we must receive your enrollment deposit. Use this form instead of the online Student Center if you intend to pay by check or money order, you are requesting a waiver due to financial hardship, or you do not need to pay the deposit since you are receiving a full-tuition scholarship.
