🤔 What does “integrated care” mean for #diabetes?    🔎 It means coordinated access to the full range of services people living with diabetes (PwD) need to manage their condition — from self-management education and psychological support to regular follow-ups and screening for complications — all delivered by a multidisciplinary team and supported by digital tools.
