I am pleased to share our most recent publication, where we examine the complex interplay between epigenetic regulation and autophagy in cancer progression. In this review, we discuss how tumors exploit the autophagy machinery—normally a homeostatic process that protects against transformation—to adapt under stress and resist conventional therapies. We focus on the role of DNA methylation, histone modifications, and non-coding RNAs in shaping autophagy-related gene expression and how this dual role of autophagy complicates cancer treatment strategies. We also highlight the therapeutic promise of combining epigenetic modulators (such as HDAC and DNMT inhibitors) with autophagy regulators to counteract multidrug resistance. Importantly, we argue for personalized medicine approaches that balance autophagic activity in order to improve treatment efficacy and patient outcomes. This work reflects a collaborative effort with my esteemed colleagues Dr. Amir Barzegar Behrooz🎗️ and Dr. Marco Cordani, whose expertise and insights were invaluable in shaping the depth of this article. 🔬 𝐂𝐀𝐑-𝐓 𝐂𝐞𝐥𝐥 𝐓𝐡𝐞𝐫𝐚𝐩𝐲: 𝐏𝐫𝐨𝐠𝐫𝐞𝐬𝐬 𝐚𝐧𝐝 𝐂𝐡𝐚𝐥𝐥𝐞𝐧𝐠𝐞𝐬 𝐢𝐧 𝐂𝐚𝐧𝐜𝐞𝐫 𝐓𝐫𝐞𝐚𝐭𝐦𝐞𝐧𝐭 CAR-T cells have redefined treatment paradigms for relapsed or refractory B-cell malignancies and multiple myeloma, demonstrating unprecedented response rates. Six FDA- and EMA-approved therapies now target CD19 or BCMA, with durable outcomes in certain hematologic cancers. 𝐇𝐨𝐰𝐞𝐯𝐞𝐫, 𝐬𝐢𝐠𝐧𝐢𝐟𝐢𝐜𝐚𝐧𝐭 𝐡𝐮𝐫𝐝𝐥𝐞𝐬 𝐫𝐞𝐦𝐚𝐢𝐧: • 𝐴𝑐𝑢𝑡𝑒 𝑀𝑦𝑒𝑙𝑜𝑖𝑑 𝐿𝑒𝑢𝑘𝑒𝑚𝑖𝑎: No CAR-T therapy is approved, largely due to the lack of a safe and specific target antigen. Most AML markers overlap with healthy hematopoietic stem cells, posing high risks of on-target/off-tumor toxicity such as life-threatening myeloablation. • 𝑆𝑜𝑙𝑖𝑑 𝑇𝑢𝑚𝑜𝑟𝑠: CAR-T cells face poor infiltration, antigen heterogeneity, and suppression by the tumor microenvironment, limiting efficacy. • 𝑇𝑜𝑥𝑖𝑐𝑖𝑡𝑖𝑒𝑠: Cytokine release syndrome (CRS) and immune effector cell-associated neurotoxicity syndrome (ICANS) remain key safety concerns.
