setting "localhost" in the mysql.yaml file may produce this error:

the workaround is to grant permissions to datadog@127.0.0.1 , rather than datadog@localhost

That is a well known issue when setting up MySQL, localhost will use the
local Unix socket to connect to the daemon and 127.0.0.1 will use TCP.

It is something referenced in MySQL documentation we could give a link to
that, but that's probably not due to the agent.

A MySQL client on Unix can connect to the mysqld server in two different ways: By using a Unix socket file to connect through a file in the file system (default /tmp/mysql.sock), or by using TCP/IP, which connects through a port number. A Unix socket file connection is faster than TCP/IP, but can be used only when connecting to a server on the same computer. A Unix socket file is used if you do not specify a host name or if you specify the special host name localhost.

So I guess they may have a special configuration where they disabled the Unix socket, which is by default enabled.

Also to make the agent connect using the Unix socket you must use that option in the configuration:
