SkeletonContainer not shown for last column in a hierarchy

Bug report

When the loaderType is set to "skeleton" and the grid is a hierarchy, the skeleton placeholder for the last column is not rendered.

Reproduction of the problem

1. Set the loaderType to "skeleton".

2. Set a child grid to create a hierarchy.

3. Load the grid.

4. The last column has no skeleton container.

The _buildSkeleton() function takes into account only the visible bound columns. An additional check has to be performed to see if the grid is in a hierarchy. If yes, either offset the columns in which the skeleton container is shown or add the hierarchy column to the visible columns array over which the HTML is built.
