Despite ongoing advancements in healthcare, systemic inefficiencies persist, leading to inappropriate points of care utilization and over-reliance on hospitalizations and Emergency Department (ED) visits. This decades-old issue continues to affect frail, elderly, and complex patients who struggle to access suitable care outside of traditional hospital settings.     The ongoing government shutdown has interrupted hospital-at-home programs across the nation, but there are alternatives for health systems looking for more sustainable options to support care at home. myLaurel operates outside of the CMS Waiver with an outcomes-based payment model. That’s why Maimonides Health partners with myLaurel to provide wraparound services that connect hospital care to primary care for high-risk patients, ensuring a seamless transition and stronger recovery at home. As Dr. John Marshall, EVP and CMO at Maimonides Health, shared: “When patients are discharged from a hospital to the home, there is a lot of uncertainty. Patients are worried. They want to know that they are safe. Their session, “The New Front Door: Developing New Gateways for Observation Services and Hospital Care,” sparked thoughtful discussion and strong audience engagement. Discussion topics included: ▪️ Operational opportunities to improve capacity, throughput, and transitions ▪️ An innovative approach to shifting care into the home ▪️ Clinical, financial, and patient outcomes from new models of care ▪️ What’s on the horizon for expanding care in the home Audience questions highlighted both the challenges and the momentum health systems are experiencing as they rethink hospital capacity and explore scalable models for high-acuity care at home. The session closed with excellent audience questions that underscored the urgency—and opportunity—of rethinking how and where care is delivered.

In her role as Chief Strategic Partnerships Officer at Aegis Ventures, she leads the Digital Consortium, a first-of-its-kind venture studio model co-developed with 14 health systems representing over $100B in annual revenue. In this role, she's responsible for driving cross-sector partnerships, accelerating commercialization, and building the infrastructure to launch Al-powered healthcare companies at scale.
