Reaching every person in your population, regardless of Zip Code, Chronic Disease, or Transportation Barriers, should be a top priority of every #healthsystem and #managedcareorganization. That distance is growing as more rural hospitals and clinics close, many operating on razor-thin margins with aging infrastructure and too few clinicians to sustain services." Action is needed.
