Congestion remains the leading cause of heart failure hospitalizations, but existing remote monitoring tools often lack accuracy. The COMMUNITY-HF study, presented by Dr. William Abraham at #HFSA2025, explored a new frontier — voice-based monitoring using the Cordio HearO™ system. By analyzing subtle vocal changes linked to pulmonary fluid overload, the system detected >85% of heart failure events, often ~25 days before clinical presentation, with a low false alert rate and strong patient adherence. AI Improves Diagnostic Accuracy in Sydney EDs New research from Sydney emergency departments shows that an AI-assisted chest X-ray tool can meaningfully improve diagnostic accuracy in the ED preventing one misdiagnosis for every 17 patients reviewed. Key takeaways: * Up to 12% improvement in diagnoses overall, with the biggest gains (~11.8%) among doctors with ~3 years’ experience. * Study involved 200 clinicians across five Sydney EDs, using a tool developed by Australian company Harrison.ai. * The trial deliberately included cases known to trigger AI false positives/negatives—offering rare insight into how clinicians respond when AI is wrong. Even marginal gains matter in high-stakes conditions like pneumonia, pneumothorax, trauma, and heart failure. In a typical ED shift reading 30–40 scans, reducing even a couple of misdiagnoses can be life-changing for patients. This is evidence of AI working as intended: augmenting (not replacing) clinicians, especially in high-pressure environments. The platform is already in use across parts of NSW and by major imaging providers (I-MED, Harbour Radiology, Sonic Healthcare). Kudos to Prof. Farah Magrabi and team for a rigorous, clinician-in-the-loop evaluation, and to Prof. Michael Dinh and RPA for leading real-world trials. A great couple of days at #HFSA2025 in Minneapolis. A few highlights and reflections from the cardiology community: - Heart failure burden: Latest HFSA stats show heart failure (#HF) accounts for 45% of CV deaths—over 425,000 annually in the US. Disparities in care remain significant, with calls for better implementation of guidelines. - Treatment updates: Encouraging data on Acoramidis in ATTR-CM impact as soon as 1-month after starting treatment, and on Vutrisiran (Amvuttra) improving GI-related quality of life. Myosin inhibitors continue to show promise in shifting the trajectory of HCM. - AI in heart failure: Several sessions touched on AI adoption, particularly LLM utilization. Many clinicians seemed early in their AI journey—underscoring the opportunity for broader education and adoption. I expected to hear more on AI on images, but it was not mentioned much at all. My takeaways: - The need for improved HF care is only accelerating as our population ages. - I still enjoy a deep MOA discussion (RNAi, etc.), and seeing advances in therapies that go beyond symptom management toward modifying disease course. - Advanced AI is still limited use, even with @Viz.ai’s success in #HCM detection on ECGs. It has so much more potential if/when we scale innovation into real clinical workflows—not just academic pilots. I'm looking forward to the next wave of echo-based scale up of AI use. - Clinical impact requires much more than a high algo PPV; it depends on workflow integration, ease of use, transparency of logic and organizational change management. Ultimately, AI in cardiology isn’t about “dropping in an algorithm”—it’s about reshaping workflows and supporting complex decision-making to drive meaningful patient outcomes. The cardiology market doesn't seem to be thinking this way. And yet there is great precedence and success from organizations like Viz that have refined our model for sustainable and scalable adoption of AI capabilities. Two milestones stood out this year: • HeartFlow’s IPO, backed by strong revenue, FDA clearance, and reimbursement — showing that AI-driven FFRCT analysis can succeed both clinically and commercially. • Tempus, building on its oncology foundation, gained FDA clearance for its cardiac MR platform, expanding its AI infrastructure into cardiovascular care. ✨ The exciting part? ⚖️ As regulators, payors, and clinicians have already been “trained” through years of oncology AI adoption, this new wave in cardiology might move faster than expected. 💊 With the GLP-1 revolution reshaping how we think about metabolic disease, it’s only natural that AI and therapeutics start converging — and could easily ride this spillover momentum. 🚧 Of course, cardiology isn’t simple either — hemodynamics, comorbidities, and imaging variability still make real-world AI tricky.
