Restoring Mobility after Knee Injury One of my recent patients came with persistent knee pain following a sports injury. The MRI showed a partial ACL tear with mild joint effusion. The patient reported difficulty walking long distances and limited confidence in daily activities. Assessment •Reduced knee flexion and extension •Quadriceps weakness •Pain during weight-bearing activities Plan of Care: •Pain management techniques and manual therapy •Progressive strengthening for quadriceps and hamstrings •Neuromuscular re-education to improve balance and stability •Home exercise program for long-term self-management After 12 sessions, the patient regained nearly full range of motion, improved muscle strength, and returned to light sports activities with reduced pain. 👉 When your patient walks in completely bent forward… It’s something we all see from time to time a patient who suddenly can’t stand upright anymore, their back locked in a painful flexed position. That’s what we call an acute kyphotic deformity. The common reflex? Try to “unlock” the spine immediately with repeated extension in prone. But very often… it doesn’t work. The patient simply can’t tolerate it yet and forcing it usually makes things worse. At this stage, the key is static positioning and patience. I usually place the patient prone, supported with one or more pillows, monitor their pain response, and wait. Gradually, you remove a pillow or raise the table section small, progressive changes over 20 to 40 minutes. That’s how you allow the deformity to reduce naturally. The first pitfall comes when the patient gets off the table — they need to move slowly, stay within the safe range, and keep moving once upright. Hello my connections...... In this article I detailed explain about Thompson test. The Thompson test is a clinical exam to diagnose an Achilles tendon rupture by squeezing the patient's calf while they are lying on their stomach with their feet hanging off a table. A positive result, indicating a rupture, is the absence of plantar flexion (foot movement downward) when the calf is squeezed. A normal, or negative, result is when the foot plantar-flexes as expected. How the test is performed Position the patient: The patient lies on their stomach (prone position) with their feet extending past the edge of the examination table or chair. Their legs should be relaxed. Squeeze the calf: The examiner firmly squeezes the calf muscles. Observe the foot: The examiner observes the foot's position for a response. Negative result: If the patient has an intact Achilles tendon, squeezing the calf will cause the calf muscle to contract and pull on the tendon, resulting in the foot automatically pointing downward (plantar flexion). Positive result: If the patient has a complete Achilles tendon rupture, there will be no or very little movement of the foot when the calf is squeezed. This absence of plantar flexion is a positive sign, indicating a tear in the tendon. Accuracy and limitations The Thompson test is a highly accurate test for diagnosing an acute Achilles tendon rupture, with a reported sensitivity of 96% and specificity of 93% in some reviews. While accurate for complete ruptures, it may be less reliable for partial tears. It's also a good first step, but doctors may still order imaging like an ultrasound or MRI to get a definitive picture of the injury. #snsinstitutions #snsdesignthinking #designthinkers #snscop #article
