Day 7: Impact Spotlight How digital discharge is already delivering — and what we’re learning.  I’m excited to share some early insight from one of our live digital discharge pilots, delivered in collaboration with local NHS and care partners. Together, we’ve:  Introduced remote monitoring and wraparound support for a seamless discharge and reablement at home  Used connected technology to escalate care where needed, reducing avoidable readmissions  Embedded data sharing to help clinicians and social care teams coordinate more proactively In a region with 97% gigabit broadband coverage, this is a real example of how infrastructure supports smarter, safer services. And this is just the beginning. Jorge Bravo, researcher at CHRC – Comprehensive Health Research Centre, is leading an ambitious initiative in the Alentejo: the Community Cardiac Rehabilitation Programme. His work aims to empower patients to better self-manage key health indicators and promote sustainable recovery. Built on the foundations of the HE@RT.FIT project — which integrates remote, real-time supervised exercise sessions via live streaming and continuous monitoring via app — this community-based programme bridges hospital care and everyday life. There will be no C-DIAS PSMG Virtual Grand Rounds next week. 📅 Join us on Tuesday, October 14, for a presentation on scaling the Collaborative Care Model by Allison Carroll and Meredith Boyd from Northwestern University - The Feinberg School of Medicine; and Emily Fu, PhD MPH from The University of Chicago. The Collaborative Care Model (CoCM) is an evidence-based intervention used to treat mild to moderate mental health conditions in primary care settings. Although researchers have extensively studied the effectiveness of CoCM, less is known about the strategies required to successfully implement and sustain the model, especially under pragmatic conditions. In this presentation, the researchers share findings from multiple phases of CoCM implementation. First, they conducted a randomized roll-out implementation optimization type 2 hybrid effectiveness-implementation trial across 11 clinics within one region of a health system. Second, they describe how lessons from this phase informed the health system-wide implementation of CoCM across 74 clinics. Finally, they present novel, rapid approaches currently being piloted to increase the adoption and reach of CoCM. I analyzed global data on cardiac care access, and the findings surprised me. Some upper-middle-income countries are catching up fast, even surpassing high-income nations in the number of coronary care units per million people. Yet, the data also revealed something intriguing: low-income countries appear to have the lowest cardiac death rates. At first glance, it looks positive but in reality, this may reflect reporting bias, limited diagnostic infrastructure, and undocumented deaths due to under-resourced health systems. When data is incomplete, inequality hides in plain sight. This dashboard is part of my ongoing project, “Global Access Gap & Equipment Demand Prediction,” where I combine R + Tableau to uncover how disparities in medical infrastructure shape outcomes worldwide.
