𝐅𝐔𝐋𝐋𝐘 𝐂𝐔𝐒𝐓𝐎𝐌 𝐓𝐊𝐀: 𝐇𝐨𝐰 𝐝𝐨 𝐲𝐨𝐮 𝐞𝐯𝐚𝐥𝐮𝐚𝐭𝐞 𝐧𝐞𝐰 𝐭𝐞𝐜𝐡𝐧𝐨𝐥𝐨𝐠𝐲? I recently did a series of fully custom total knee arthroplasties with restor3d. Here's how I evaluated the technology in my practice: 1. 𝐊𝐍𝐎𝐖 𝐘𝐎𝐔𝐑 𝐖𝐇𝐘 ➡️ New tech must be cheaper, faster in the OR, or improve patient outcomes. Trade-offs in any of these three categories must be balanced by benefit in the other two areas 💡 Custom TKA was time neutral by my 4th case, cheaper overall, and will hopefully improve patient outcomes 2. 𝐃𝐄𝐃𝐈𝐂𝐀𝐓𝐄 𝐓𝐈𝐌𝐄 ➡️ Don't wait until the end of the OR day when your patience is thin to try something new. 💡 I scheduled a day with 4 left knees: standard varus, bad varus, standard valgus, bad valgus to put the technology to the test. It performed best in the bad valgus knee! 3. 𝐏𝐀𝐓𝐈𝐄𝐍𝐓 𝐁𝐔𝐘-𝐈𝐍 ➡️ Would you want the new technology used on your knee? Involve patients in your decision and let them know they are the first to recieve it 💡 Patients jumped at the opportunity for a full-custom knee replacement and were excited to be the first in my practice What other things do you look for when evaluating new technology in your practice?
How did you find timing from consult to OR? I love PSI for offloading intra-op thinking / time and equipment reduction. Quicker surgeries, with quicker turn arounds and some of my happier patients. Agree with Brian Fuller, it didn’t make sense for a teaching institution as most of the positioning and thinking was done by the time the case started. What did you do for the lateral condyle in the bad valgus case? How much did you build back posteriorly and what were you using to reference?
Great video Tim Keating, M.D. And Instead of having to “sell” PSI to your patients, what if they came to you WANTING IT because they KNEW you were the specialist that does this well? They have a huge marketing advantage over off the shelf.

Your process is the right one. It's also the one most systems are designed to prevent, because they prioritize throughput over the deliberate focus required to properly integrate something new.
Great framework for assessing new tech, Tim Keating, M.D. I like the emphasis on balancing cost, efficiency, and outcomes. Too often one gets prioritized at the expense of the others. The structured test day with different knee types is a smart way to stress-test the system.

2moSimilar to other tech, definitely a marketing advantage. I hate PSI for teaching/learning, but love it for efficiency, especially at ASC. I was excited to try iKA with MAKO and trial wirh Verasense several years ago. I was not time neutral, it was not cheaper, and patients didn’t do any better. Stryker made it easy to abandon by shelving the Verasense
