1. Reference: Memo from the National Headquarters dated 03 January 2017; received by
this office (Operations Division, BFP8) dated 09 January 2017.
2. ICOW the above-cited reference, you are hereby directed to use the attached format of
the Affidavit of Undertaking to be submitted by the applicant as one of the documentary
requirements in order that an Occupancy Permit maybe used as basis for the issuance
of FSIC for Business Permit pursuant to afore-cited Memorandum (Annex A).
