of TUKTUKAN, GUIGUINTO, BULACAN do hereby NAME APPOINT AND CONSITUTE the name of
ERNESTO ALBANIA, single, of legal age, Filipino and a resident of 394 TUKTUKAN, GUIGUINTO,
