III. Improving performance


1. Copy of logbook for TB referral
2. Copy of the following:
- Accountant or accountant clerk – Diploma(if applicable), certificate of trainings attended, proof of
employment/Appointment

PHYSICAL PLANT REQUIREMENT


1. Provision of PWD toilet
2. Functional Delivery and Labor Room

1. Examining table with stir-up (Emergency Room)
2. Oxygen tank unit with tank holder, BP apparatus and thermometer (Labor Room)
3. Haloperidol 50mg/ampule
4. Airway adjuncts
