AFFIDAVIT OF DESTRUCTION
(OF CASH BOND)

I, JOCELYN TALINGTING LEELIAN, of legal age, Filipino, and


a resident of A. Del Rosario St., Flores Compd., Guizo, Mandaue
City, Cebu, after having been sworn to in accordance with law, hereby
depose and say:

1. That I am an Accounting Assistant of Metro Retail Stores


Group Inc., a corporation duly organized and existing under
and by virtue of the laws of the Republic of the Philippines;

2. That as the Accounting Assistant of the said company, I had


once in my possession a cash bond in the amount of
PESOS: NINE THOUSAND (P 9,000.00) which I kept in the
office at Metro Ayala;

3. That sometime in January 2020, Metro Ayala was burned


down;

4. That the said cash bond was destroyed when Metro Ayala
was burned down;

5. I am executing this affidavit of loss to attest to the truth and


existence of the foregoing facts;

6. Affiant further sayeth naught.

IN WITNESS WHEREOF, I hereunto set my hand this


November 28, 2019, at Mandaue City, Philippines.
