AFFIDAVIT OF LOSS

I, CIRILO E. SARDIDO, Filipino, of legal age, single, and a


resident of Cabligan, Matanao, Davao del Sur, Philippines, after being duly
sworn to in accordance with law, hereby depose and state:

That on January 6, 2020 at around 9:00 o’clock in the morning, I went


home after going to Digos Public Market and it was then that I realized that I
have lost my wallet;

That inside the said wallet is my Driver's License, original copy of the


OR/CR of my motor vehicle, PHILHEALTH ID and PAG IBIG identification card
among others;

  That despite diligent search and efforts to locate the said wallet,


OR/CR and my identification cards, I could not find them such that I now
believe that they are now lost beyond recovery;

That my Driver's License has not been confiscated by the LTO, Police


or other Traffic Enforcers for any traffic violation.

As such, I am executing this Affidavit of Loss to attest to the


truth of the foregoing and to support my application for the issuance of a
new Driver's License, new OR/CR and new identification cards, in lieu of the
ones that were lost.
