1.0 Introduction
1.1 QP advised TADMUR that the Effective Date of the Contract is TADMUR
01.08.2011 and Completion Date is 30.06.2013 (23 months).
1.2 QP advised TADMUR that the shutdown period will as per the contract INFO
document, i.e. 07 April 2013 to 20 April 2013.
Safety
2.0 INFO
QP reminded TADMUR that there will be no compromise on safety
and it should always be a top priority.
QP Highlighted that the dusts control is not up to the QP
requirements and advised Tadmur to remove the excavated material
and replace the soil with Good Grade B soil.
2.0.1 TADMUR
TADMUR informed QP that excavated material will be removed
within 10 days, starting from 2-June-2012.
QP asked TADMUR to clear the punch list raised by HSS (D), during
Monthly Inspection.
2.0.2 TADMUR
Tadmur replied that the Punch list will be cleared and submit the
status/report to QP on or before 4-June-2012.
2.1 Safety Officer
2.1.1 QP has stated that safety officer shall not be used for permit INFO
coordination and a separate permit coordinator shall be deployed for
permit coordination.

2.1.2 QP informed TADMUR that the Tool Box Meeting Topics that were TADMUR
discussed for the last two weeks shall be summarized and shall be
included in the weekly progress MOM .

QP informed TADMUR that weekly safety statistics has to be

QP advised TADMUR to submit the monthly man hours for each
month and this report shall be sent to EDD/5 Office on the first

Security Pass
2.2 INFO
QP advised that Security Pass would need to be obtained for workers
planning to work at site.
Equipment
QP informed Tadmur that all equipment including lifting equipment
used by TADMUR needs to be approved / certified by 3rd party and

Security Pass would need to be obtained for all equipment and
vehicles. Also fitness of the vehicle and equipments needs to be
approved by QP safety officer. Also QP advised Tadmur to revise the
lifting plan and resubmit.
2.4 Work Permits
2.4.1 QP advised that Work Permits and Excavation Permits shall be TADMUR
obtained by TADMUR pertaining to particular work and location prior
to start an activity.JSA and AFC drawings needs to be attached with
the Work permit
2.4.2 QP reminded TADMUR that it is important that adequate site TADMUR
supervision is to be provided. It is unacceptable for work sites to
operate without properly trained supervisors.
2.4.3 QP has advised TADMUR that copies of approved / required docs TADMUR
shall be attached to their respective permits and all the open permits
shall be signed off and closed by the permit holder.
Scaffolding
QP stressed on the quality of scaffolding to be used. All scaffolding
shall be as per applicable BS standards and QP requirements. All

scaffolders shall be certified and approved by Third Party. Qualified
personnel shall carry out dismantling and erection of scaffolding.
Qualified scaffolding inspector has to be available to sign the
scaffolding tags.
Tool Box Meetings
QP advised that Tool Box Meetings shall be held daily and that each
week a list of tool box agendas and list of attendees needs to be

recorded, tracked and kept at TADMUR site office.
Log of tool box meeting shall be maintained at site with all attendees
signatures.
2.7 QP informed that Weekly Safety Audits are conducted at site and INFO
potential hazards are identified and on-going construction activities
are monitored for compliance to safety rules and regulations. No
tolerance is exercised on observing carelessness to safety.

Incident and Near Misses Reporting
All incidents shall be duly reported and TADMUR is encouraged to
report all near misses encountered to site project group. EDD/533 to
be informed by the fastest practical means.

In case of incident, it has to be reported by Tadmur in the following
sequence;

Incident report to be prepared and submitted to QP within 24 Hours
via email addressing to focal point (EDD/533) and Cc To EDD/23 &

Personnel Protective Equipment
PPE (Personnel Protective Equipment) is mandatory to be worn at
site at all times by all staff. As a minimum, PPE comprises of
2.10 coverall, hardhat, safety boots (steel toe) and eye safety glasses. INFO
Additional protective equipment shall be supplied and worn on as
required by the job task. Portable H2S monitors shall be provided
and harness shall be provided for personnel working above 2.5 m
height.
3.0 Communication
QP briefed TADMUR regarding communication related to field based
construction activities including site survey, all correspondences with
attachment shall be sent to QP Lead Construction Supervisor, Mr.

4.0 Mobilization
Mobilization Plan – TADMUR informed QP that it has been updated

5.0 Planning
Construction Progress Monitoring
Weekly progress meeting to review and discuss the construction and INFO
engineering progress status will be regularly.
5.1 Once the site facilities are available, weekly meetings will be INFO
conducted on site.
TADMUR shall prepare and submit minutes MOM for WPM, within 2 INFO
working days for QP review / comment. All minutes shall be mutually
agreed and signed by both parties.
Progress Schedule and Reporting
TADMUR shall submit Weekly Progress Report (WPR) on Sunday of
every week latest by 7:00 a.m., Cut-off day shall be every Thursday of
the week. WPR, as a minimum, shall include: TADMUR
- List and status of activities (Construction) carried out during the past
week, INFO
- Actual versus planned progress,
- Area of concerns (pending and new issues),
- Three week look ahead (past week, present week and following
week).
5.2
QP advised that TADMUR to provide 2-week look ahead in the
weekly progress reports (with a 1 week look back) which will include TADMUR
activities completed last week, activities being done at present week
and planned activities for the following week.
QP advised TADMUR to provide details about man-hours worked,
incidents, LTI (Lost Time Incident) etc in the weekly reports. TADMUR
TADMUR to send Fax on the first of every month with the monthly
man-hours for the previous month.
TADMUR
Three week look ahead schedule shall be submitted to EDD/533 on
weekly basis.

6.0 QA/QC
Procedures and Plan Approval
6.1 QP advised TADMUR to submit the ITP for all major Works before 5- TADMUR
June-2012. TADMUR agreed on the same.
Method Statements for Major Works
6.2 TADMUR shall submit method statements for all major works TADMUR
including critical / heavy lifts and as advised by QP prior to
commence the work.

TADMUR to submit method statements; hazard identification etc for

TADMUR informed that the above list of sub-contractors are already
working on site.
8.0 Site Working Hours
TADMUR should plan their work from 6:30am to 2:30pm, 5 days a
week. No work will be allowed on Fridays and Saturdays. However,
8.1 QP will allow extra work hours during daylight hours, but any work INFO
done outside the 6.30am to 2.30pm zone or Saturdays, needs to be
pre-approved by QP.
In addition to the above, QP advised Tadmur to follow QP Fax ref
no.GC10106300/F-001, dt.22/09/2011 issued to Tadmur.
8.2 With reference to above fax, TADMUR to be allowed to work at site INFO
until 4:30 PM, Sunday to Thursday including Saturday except
Fridays & Holidays.
QP advised TADMUR to apply to EDD/5 requesting Green Field
Status for Jaleha New Control Building Area on urgent basis.
8.3 However, excavation permits shall be obtained for all the external TADMUR
excavation activities such as cable trench works, Earth pits,
Underground utility works etc;
