AFFIDAVIT OF LOSS

I, EDMOND L. LAGMAN, of legal age, single, Filipino and a resident of #025


Sta. Filomena Guagua, Pampanga, Philippines after having been duly sworn in
accordance with law, hereby depose and state:

That I am a holder of certain Professional Driver’s License issued by the Land


Transportation Office (LTO) at San Fernando Pampanga;

That the said Driver’s License was inside my wallet together with other
important documents and ID’s and few amounts of money;

That on December 2012 while I was walking in the street of Sasmuan,


Pampanga, I notice that my wallet where I put the said Driver’s License is missing in
the back pocket of my pants;

That despite diligent search and efforts to locate the said wallet, I could not find
it such that I now believe that it is now lost and beyond recovery;

That my Professional Driver’s License was not confiscated by the LTO, Police
or the other Traffic Enforcers for any traffic violation;

As such, I am executing this Affidavit of Loss to attest to the truth of foregoing


and to support my application for the issuance of a new Professional Driver’s License,
in lieu of the lost one.

IN WITNESS WHEREOF, I have hereunto set my hand this 28 th day of January,


2013 at Guagua, Pampanga.
