I. Introduction:

Covid-19 Pandemic brought many changes and challenges in the Philippine education system
but we, Guiguinto District was able to cope with these circumstances.
Health Education emphasizes the development of positive health attitudes and relevant
skills in order to achieve a good quality living. In this time of Covid-19 Pandemic, Health Education is
an important component in the prevention of this deadly disease.

This report intends to:

1. Give information about achievements/ accomplishments made by the


Health teachers and the Pupils.
2. Identify the activities undertaken to achieve the desire target for the School Year
2021-2022.
3. Express future plans for the subject Health

A. Pupil Development

1. Conducted a diagnostic test in Health subject.


2 .Implemented different intervention programs and activities with the help of
the pupils’ parents and guardians.
3. Created enough instructional and supplementary materials for pupils in Health.
4. Enhanced and developed pupils’ skills and talents with the guidance of their
Parents and guardians.

B. Teacher Development

1. Attended the District Online meeting of MAPEH Coordinator.


2. Division Virtual Meeting of MAPEH advisers, Coordinators and MAPEH
Department Heads for National Arts Month Celebration

C. Curriculum Development

1. Finished/ completed the required learning competencies for each grade level
during the 1ST quarter up to 4TH quarter.
2. Assessed pupils’ performance through different tests and performance tasks.
3. Continuous preparation of Instructional Materials.
4. Created and distributed self learning modules (SLM), weekly home learning plan
(WHLP) with answer sheets and learning activity sheets (LAS)

D. Accomplishments:

1. Last August 2021, The MAPEH Group headed by Mrs. Rosario L. Martin and
Mrs. Erlinda Tolentino, crafted an action plan for the School Year 2021-2022 to
organize the district improvement effort.
2. In pursuant to the Division Memorandum no. 163 s. 2021 of the Schools Division
of Bulacan , One Health Week “ Bayanihan para sa kalusugan: OK sa DepEd sa
Paaralan at tahanan” the District of Guiguinto headed by Mr. Gilbert M.
Agapito,District Supervisor, together with the supervision of Mrs .Rosario L.
Martin,the District Health Adviser in coordination of Mrs. Allaine M. Cortez ,District
Health Coordinator ,Guiguinto District conducted a culminating activity last
September 10,2021 at 3 o’clock in the afternoon via Facebook livestream through
the DepEd Tayo Guiguinto FB page.

3. Last February 15, 2022, the MAPEH Group attended the Division Virtual Meeting of MAPEH
advisers, Coordinators and MAPEH Department Heads for National Arts Month Celebration
4. Last April 19, 2022, the MAPEH Group had a Consultative Meeting regarding identification of 4C’s
in the MELC’s and crafting of MAPEH summative tests.

5. The well- trained District writers constructed MAPEH summative tests for the
1st quarter up to 4th quarter.
6. The Health group was able to Identify the most and least learned competencies in the
MELC’S from 1st Quarter up to 4th quarter.

7. The Health Group identified the 4C’s in the MELC’s of grades 1- 6 from quarter up to quarter 4
