PURPOS! 14 This Safe Operating Procedure recommends safe practices, steps and measures to be taken in protecting workers from the hazards involved in Excavation & Trenching to avoid “Personal Injuries, Occupational Diseases and environment damages’ in line with EHS Objective of “Zero Harm’ across the sites. 1.2, It also describes the steps and measures for COVID-19/ Pandemic outbreak and preparedness. 2. TERMS & DEFINITION: a. Excavation: Excavation is man-made cut, cavity, trench or depression in earth surface formed by earth removal. b. Excavation is the process of moving earth, rock or other materials with tools, equipment, or explosives. It also includes trenching, wall shafts, tunneling and underground. Itis the preliminary activity of the construction project ¢. Trench: A narrow excavation that is long, narrow (generally), and deeper than its width. But the width of a trench is not greater than 4.5 Mt. (15 feet) at the bottom. . Ramp: An inclined surface for movement of people /Vehicles. e. Angle of repose: The greatest angle above the horizontal plane at which the material lies without sliding f. Cave-in: Separation / sliding of mass of soil or rock material from the side of an g. Shoring: Temporary structure that supports the sides of an excavation and protects against cave -in h. Sloping: Angle of inclination on the sides of the excavation. i. Competent Person: A competent person as someone who is: capable of identifying existing and predictable hazards in the surroundings, or. working conditions which are unsanitary, hazardous, or dangerous to employees, and. who has authorization to take prompt corrective measures to eliminate them. 24. Classification of Excavation Excavation can be classified into below category: 1. Sequence of Activities a Before start of work, permission to be obtained from the authorities having jurisdiction. Trials Pits: Trial pits shall be excavated manually before using the equipment to identify the underground utilities. Workmen involved shall essentially use safety shoes, electrical safety hand gloves and insulated crowbars & other tools. 3. Also, use GPR drawing for underground utilities. Above ground utilities survey: The site engineer shall look for the presence of above ground utilities by physical site visit survey. 1. Prior to start of excavation work, Crack survey of existing structure should be carried and recorded with photographic evidence 2. Hazard Identification and Risk Assessment must be carried out prior to the work commencing. 3. Subsidence and Bulging: An unsupported excavation ‘can create an unbalanced stress in the soil, which in tum causes subsidence at the surface and bulging of the vertical face of the trench. Project Manager: a. The Project Manager shall demonstrate the commitment in managing safety and health and, bear the final responsibility and accountability for the safety and health in the industrial undertaking b. Shall ensure a safe and healthy working environment. c. Shall provide the necessary resources for managing safety and health at the site. d. Shall ensure the experience and trainings of employee is commensurate with the employee's assigned task. e. Shall ensure that the roles and responsibilities for managing safety and health are assigned, communicated throughout the levels and carried out effectively. f. Shall ensure complete investigation of all dangerous occurrences and accidents and, recommendations shall be followed-up. 4.2. Line Management: a. The line management personnel, Section/ Site-in-Charge, Zone In-charge, Site Engineers have a supervisory role and are responsible for managing safety and health at the site on a day-to-day basis. aye b. Shall ensure the frequent inspection and maintenance of all equipment before and during the operations. d. Shall ensure the usage of necessary safety equipment and personal protective equipment for the assigned tasks. e. Shall report all the hazards, incidents, injuries, and accidents, defects found in equipment, PPEs etc. and deviations during daily operation to the management immediately. f. Shall understand and act in accordance with the safety rules, safe working procedures and emergency response procedures. 4.3. EHSO: a. Shall ensure that the experience and training of the employee commensurate with the employee's assigned task. b. Shall ensure that Safety Rules and Safe Working Procedures are made available, understood and, complied with relevant legislation. c. Shall ensure that the risk assessment of operation is carried out and, safety measures are established d. Shall ensure regular and frequent check and maintenance of all the equipment being used in the tasks. e. PRECAUTIONARY MEASURES: a. If dewatering pump is operated electrically, the supply shall be routed through RCCB of 30 mA and the pump shall be earthed b. Before commencing excavation work, the Site Engineer shalll look for the presence of underground utilities using detectors and checking for the presence of markers. ©. Trial pits shall be excavated manually before using the equipment to identify the underground utilities. Workmen involved shall essentially use safety shoes, electrical safety hand gloves and insulated crowbars & other tools. d. Work permit shall be taken from the owner of the utility, the utility shall be de- energized/emptied and all the safety requirements mentioned in the permit shall be complied with. e. In case of underground electrical cables, the cable shall be de-energized and if the excavation is to be done manually, the tools like crowbar, spade, etc. shall be well insulated to prevent electrical shook. f. If dewatering pump is operated using Engine, refueling shall be done only when the engine not running. g. Allrotating and moving parts of the dewatering pump shall be guarded. h. Dewatering pump shall be secured so that it does not fall inside the trench. It shall be placed on flat surface at safe distance from the excavation. Fig. Barrication for Excavated Area 61. Movement of vehicle and heavy cranes shall be 3 feet (1Mt) away or 1.5 times the depth of excavation, whichever is greater. c. Adequate and well-anchored stop block shall be provided on the surface to prevent operating vehicles from falling accidentally into excavation pit. d. Workmen shall be provided and wear florescent jackets to control the traffic with flags. e. The trench shall be provided with hard barricade or earthen bund. Warning light/light reflectors shall be provided to warn the vehicles drivers in the night. 6.2. __ Fall of Person and Materials a. Hard barriers/earthen bund, warning signs and warning lights / ight reflectors shall be provided to prevent fall of persons/vehicles inside. , b. Provide barricading of the area'and display of warning signboard in Hindi / English / regional language at conspicuous locations. c. Waring signs including light signal to be provided. d. No trench, ditch or other excavation shall be left overnight without barricades and warning lights. . Adequate illumination should be provided in the night and in day as per site condition so that the area will become visible. f Suitable warning sign, such as fluorescent warning tapes, flashing lights and fluorescent jackets shall be provided to warn the persons in night. 6.3. Hard Barrier or Barricading: Barricading shall be done as per below guidelines- a. Hard barricading must be capable of supporting 90 kg (L&T Standard) / 100 kg (OSHA Standard) of force. b. Height of top rail of barricade must be from 950 mm to 1150 mm. Height of mid rail will half of the height of top rail. c. The distance of two vertical span is 2.4 m max. d. A minimum of 1-meter distance must be maintained between barricade and edge of opening / excavation pit. e. Length of barricading pipes should not be more than 03 meters. MS pipes must be coloured in red and white zebra strip (length of colour strip is 300 mm). Horizontal pipes should not be protruded more than 300 mm from vertical post and should not be less than 50 mm. h. The warning barricades must be 6 feet away from the edge of the excavation (plastic tape & sign board) only if depth of excavation is up to 1M i. The barricades installed closer than 6 feet (1.8Mt) from the edge of the excavation, must be hard barricade. 6.4, Earthen Bund: Earthen bund shall minimum 1 m in height and at least d/3 distance away from the ‘edge of trench / excavation pit. 1. Cross over bridge with guard rail shall be provided across the trench for the local people and animals. 2. Access egress: Ladders shall be used to enter or leave the excavation. The ladder shall extend at least 1m above the ground level. b. If the excavation is 4 feet (1.2Mt) (as per OSHA) / 1.5 m (as per BOCW Act) or deeper it should be provided with standard ladder to facilitate safe entry and exit. c. Trenches 4 feet (1.2Mt) or more in depth must have ladders spaced so that employee's lateral travel to a ladder does not exceed 25 feet (8Mt). The height of the ladder to be extended up to 3.3 feet (1Mt) from the top of ground surface. The ladder must be secured. 65. Access/ Egress at Workplace a. Width of ramp should not be less than 430 mm (as per BOCW Act) / 1.2 m (as per OSHA). . Every runways or ramp provided for use of building workers located more than 03 m above the floor or ground is on open sides provided with a guard rail of adequate strength and height of guard rails should not be less than 1000 mm. |. Overhead Electrical Cable: If overheads electrical cable comes in line/ above of excavation / pipe laying- a. Existing Structures a. No excavation or earthwork below the level of any foundation of a building or a structure shall be commenced unless adequate steps are taken against collapse of the structure or excessive settlement. b. Foundations, adjacent to and below which excavation is to be made, shall be supported by adequately designed sheet piling or shoring, as long as the excavation remains open. c. Excavated materials shall not be piled against walls as this may endanger the walls. 6.8. Cave-in Protection: a. Excavated material shall be kept away from the edge of the trench to provide a clear berm of a width of not less than one-third the final depth of excavation. b. In special cases, where the disposal area is limited or where the application of this requirement is impracticable, the Site Engineer may adopt a berm of reduced width in any case not less than ‘Im, provided the material being excavated is sufficiently stable and shoring is provided to carry the additional load. c. Excavation walls shall be dressed to remove loose materials and boulders, which can fall inside. 6.8.1. Sloping a. Where the area is not a constraint and the cost of shoring is working out much more than sloping than the banks of the excavation should be sloped. b. Sloping can be done faster as compared to shoring. c. Angle of repose is one of the important considerations to decide the slope. Shoring and Timbering a. Toavoid Cave-In, Shoring is to be done for excavation more than 1.5mtr (except stable rock) where it is not possible to maintain the safe slope as defined in the standard (Ex: space constraint), b. Shoring is to be done for even less than 1.5mtr depth of excavation if site condition so demands. . Bracing or shoring of trenches shall be carried along with the excavations, d. Shoring and timbering shall be extended 300mm above the height to prevent the fall of materials in trench, e. Type of shoring -— 1. Timber shoring 2. Sheet shoring 3. Hydraulic support shoring f. All materials such as timbers, trench sheets & props must be of sound construction. The behavior of workers, supervisors in the working area shall be monitored and behavior-based safety training to all employees shall be provided. i. Drug & Alcohol Policy and Smoke-free Policy shall be adhered. j. Use of approved personal protective equipment for different tasks as per the required standards, k.
