2023-2024 Field Practicum Supervision Note
This field course requires students to document an hour of supervision for every 30 field practicum hours accrued, using
this template, along with their timesheet. Students are required to identify and discuss the application of social work
competencies, skills, theory, knowledge, cognitive and affective processes and/or ethics acquired in the community-based
field practicum during supervision with the Field Instructor. Please see field course syllabus for more information.

1.) What practicum tasks and activities did you engage in during the above noted time frame?
During the first two weeks of my field, I worked one on one with my Task Manager. She showed me what she does
for her position of the company. She is the Director of Activities, so she’s in charge of conducting all activities and
groups. She also communicates with all the families as well. While shadowing her, I learned the process of
communicating with the residents and families, ensuring the activities are interactive and stimulating, along with being
on the floor conducting the activities or groups. I was also able to observe how she conducted group sharing as well.
While listening, I was able to see how the residents respond to her and to each other. Even when certain things were
said, I remained calm and didn’t allow my personal bias and remained non-judgmental. I was also given a training on
technology when it comes to conducting activities.

3.) What are your thoughts on your social work practice during this time (e.g., how well did you engage in assigned
tasks, engage in professional behaviors)? Please note any areas you felt you performed well and then areas you
need to improve or strengthen, need more opportunity to develop confidence or competence.
• During this timeframe, I was able to learn the other side of The Blake. Being able to interact socially with the
residents, I feel with be very insightful. Being able to shadow and ask questions when needed was amazing.
Areas I feel like I need to improve on would be communicating with the residents from an activities side versus
the caretaker side. I’m very strong when it comes to be observant and attentive.
4.) What are your field instructors’ thoughts on your holistic progress during this time? Please solicit or receive their
friendly feedback and note it below.
• Field Instructor
o Bryce: MR is doing an excellent job thus far and is displaying professional SW skills in the field. MR has
been able to navigate some difficult and potentially uncomfortable/unsettling stations at her place of
work/field and has approached it with a positive and constructive mindset. MR is very organized, on
time, and does tasks to completion.
5.) Please explain how the experiential learning in this practicum combined with your self-reflection and feedback
informs your social work practice going forward.
• Being able to be behind the scenes of things was very different for me. After being a front-line worker for so
long, it was different seeing Social Work from another viewpoint. I have a lot more appreciation for Social
Workers. Between being understaffed and underpaid for all the work that they do behind the scenes, they
deserve so much more. Social Workers really work from the heart, and they work so hard to ensure there are
plenty of resources and justice in the world. Having this experimental learning is very beneficial. It’s giving me
the steppingstones I need before going completely into the field.
