EMR – electronic medical record
Vital Examinations: this are Temperature, Blood Pressure, Pulse Rate, Weight, Height, BMI (Body
Mass Index)
HEENT Examinations: this examination will take place on the patient’s Head, Ear, Eye, Nose and
Throat.
Respiratory and Cardiovascular Examination: this examination will take place on the breathing
system and on the heart of the patient.
Gastro-Intestinal Examination- this examination will take place on the abdominal part of the
patient.
Genitourinary-this examination will take place on the urinary system of the patient.
Musculoskeletal-this examination takes place on the patient’s muscles and skeletal system.
Central Nervous System-this examination takes place on the brain and the spinal cord of the patient.

ABSTRACT
Have you ever asked yourself the question “Do I have a well collected and organized medical
history”? in Ethiopia most people would probably say “NO” because it’s true that getting a new medical
history is more convenient than finding your previous history. But now we are here to tell you that YES!
you can have your medical records organized in one place. The first question which comes to mind is “how
are you guys going to do that?”. Well its easy, it is called EMR (Electronic Medical Record). EMR is a way
of recording and storing medical record electronically. Today we are presenting a robustly constructed web
application which plans to share medical record of visiting patients with institutions of care in a manner of
concise description in which it starts from birth and continues chronologically so you can enjoy the
unparalleled control and use of your medical records securely and articulately.

1. INTRODUCTION
1.1 Background of Tikur Anbesa Hospital
Tikur Anbesa hospital was built by the contribution from the Ethiopian people and was established on
Oct.1973. At first it was named as The Memorial Hospital of Prince Mekonnon Hailesellasee. The name of
the hospital was changed to “Tikur Anbesa Hospital” in 1975. When it starts functioning on Oct.1973, it has
120 beds for patients and having all the major departments (Department of Internal Medicine, Department Of
Surgery, Department Of Pediatrics, and Department Of Gynecology).
1.2 The Existing System
The current system being used in Tikur Anbesa hospital is a file based system in which a patient
comes to the hospital, it is by either being referred to the Hospital or being in an Accident. The patient will
fill a form that describes about oneself, those like Name, Age, Sex, Address, Region, City, Sub city. Then
there is an office called “Triage”, an office which will tell the patient to which doctor to go. Once the patient
reached the doctor, the doctor will do different physical examinations that will make the doctor gather
information about the current medical status of the patients. The physical examinations that will be taking
place are:

After the doctor examined all the examinations listed above, the results will be written down on the
medical card of the patient. Then the doctor will write the analysis that he got from the exams then, he will
order required laboratory tests that the doctor want to see the results of. And the laboratory tests and the
results will be dotted down too. Then, using the physical and the laboratory examination results, the patient
will be diagnosed with a disease and treatments will start to be given to the patient.
1.3 Statement of the Problem
From what we have observed from the existing system and from the interview responses acquired at
Tikur Anbesa hospital we have consolidated the following problematic statements.
On the medical cards of the patient, the hand writings of one doctor might not be readable for the
other. So when the patient is transferred to another doctor even if it’s in the same hospital, the information
that is written won’t be valuable anymore so the new doctor is required to make all the examination listed
above, so it is a waste of time and resource. The second problem that doctors face while using the existing
system is that, as the medical card is a hard copy, it will be lost and then the race to treat the patient is back to
square one. The other problem doctors’ face, especially those working in referral hospitals like “Tikur
Anbesa” is that the patients usually are long term patients that take a longer time to treat. For these patients
their medical records, since they were admitted to the hospital to the current day while still being treated, will
be bulky and too much to read and it is not organized to do so.

1.4 Objective of the Project
1.4.1 General Objective
The objective of this project is to develop a web based EMR for the purpose of fracturing and
breaking through the basic problems encountered at medical and care facilities.
1.4.2 Specific Objective
This project will also include the following Specific Objectives.
To enable the hospital to manage their medical records efficiently.
To provide the patient with simple and elegant way of accessing their records.
To ensure a better way of communication among the doctors.
To provide non-redundant record for hospitals.
To provide the receptionist with effective and easy way of searching and storing patient records.
1.5 Proposed System
The system that is going to be developed has been implemented all over the world, especially around
developed countries with firm infrastructures and well-designed software.
An EMR is a network connected electronic system where patients come to a hospital with their
records already in store. And if a patient isn’t registered that person will take basic tests and get on the
system for long lasting days. Building on that it is not hard to imagine that this will surely assist in the
medical development sector. the system is basically related to this with some additional features and a few
tweaks and it works in a peculiar way.
The system is a web based application by which a doctor will have an account to log into the site and
search for a specific patient using the patient’s unique ID number which is given to each patient when
enrolled for the first time. The system will provide a platform for the doctor to input all the examinations
results by choosing the information required from those that are listed.

There will also be a list of every laboratory exam available and the doctor will pick the required lab
exams for the patient and order it on the system for the patient. As the patient goes to the lab technician and
gets ready to be tested, the only required thing for the patient to know his/her own id number so that the
technician can log in to the patients’ page in the web system and see the ordered lab exams by the doctor.
The system also provides a platform for the lab technician to put in the results of the lab exam on the
system. And once the medical record of a patient is saved in the system, it is available at any time for a
particular patient to visit a doctor, and if there is a change on the medical status of the patient, the doctor can
edit the status on the system. Therefore, the system will remove any type of redundancy.
1.6 Feasibility Study
1.6.1. Economic Feasibility
1.6.1.1. Developmental cost
While developing this project we are going to use free software and use
our own computers and other hardware in addition to that we are going to build
our web based application by ourselves along with the help of our advisors so
there is no need for additional man power. Therefore, the development cost is
ZERO.
1.6.1.2. Operational Cost
For this project to be up and running, we are going to have to rent a
web server to host the website and also a domain name. but after the free trial
of two years’ medical organizations are going to pay a small fee to use the
system per year which will cover implementation costs.

1.6.2. Technical Feasibility
In Tikur Anbesa Referral hospital, there are computers having internet
connection for the doctors’ in which their won’t be any additional cost to buy the
hardware’s to use the system in addition to that the doctors have a good understanding
on how to use the computer along with the internet which is enough to use the system
flawlessly so the system is technically feasible on Tikur Anbesa.
1.6.3. Schedule Feasibility
The development process of this project is dynamic but we can complete all the
features listed in section [1.7] by the time frame allocated in the project management
plan in section [1.9]
1.7 Scope
The system is developed for Tikur Anbesa referral hospital but we are hoping to extend the
service gradually to Addis Ababa and then to other regions as time goes by.
The system will also have the following features
A login page which only the doctors or the person himself can view the
medical records of oneself
It will be compatible with different screen size devices (desktop, tablet or
mobile)
Print your medical records on paper
Get a digital copy like .pdf document of your medical record
Log doctors which has accessed your medical records
Doctors can send and receive laboratory test results
Password reset option for forgotten password

1.8 Methodology
1.8.1. Data Collection Methods
We have been collecting data for the project proposal using different methods like interviewing
doctors and on our observation at the hospital.
1.8.2. Description of Data
The data handled within this project are very sensitive which require intense care and attention.
These data are private medical information of people that come to the hospital and get its service.
They vary as the number of tests taken at that hospital Regarding to the fragility of these data ,they
are only shared with certified and limited personnel.
1.8.3. Implementation
The lifecycle that is best suited for this project is the waterfall model which assists and articulates
the implementation process. The waterfall model has the following characteristics.
Requirements are very clear and specified.
Ample Resources must be available freely and in vast number.
It is good to use this data model when the technology to be implemented is understood.
The project should be short.
It requires a lot of time to spend at the beginning, not to get a step back at the middle of the
implementation.
Spending a lot of time at the beginning of the software production cycle can reduce the costs
at the later steps of the production.

Therefore, for the system to be implemented:
There are enough resources available freely.
The requirements are clearly defined.
The time required is less than 4 months, and this implies the project is a short project.
Building on the above statements the characteristics of the waterfall model matches the properties
of the system to be implanted. Relying on those facts it is plausible to visually perceive that the
waterfall model and the project are aligned linearly.

1.9.2. Quality Management Plan
During the various stages of developing this software, different quality measurement
processes will be carried out. This includes consultations with our advisors to ensure that
the pre-defined standards of documentation are met.
The testing stage of our project life cycle is expected to relentlessly measure the usability,
reliability, efficiency and understandability of the system has been implemented to the
highest degree to match our clients requirements.
1.9.3. Communication Management Plan
In respect with delivering an impeccable system that meets the requirements, this team
is structured based on the strengths of each individual in a way that would guarantee
maximum output. Periodic meetings of an hour 4 days a week will be conducted within
Addis Ababa Institute of Technology. These meetings will be well documented and
minutes will be held to help ensure the consistency with decisions that will be passed. We
expect that this will play a big role in the growth of the project and in providing the users’
of the system with one-of-a-kind experience.
