DISINFECTION PREPARATION
Prepare decontamination solution by mixing 1 part 5%
chlorine bleach to 9 parts waters to make 0.5% chlorine
solution.
Assessment
1. Makes the client comfortable

- Onset of the uterine contraction
- Frequency and regularity of uterine contraction

the progress of labor.
10. Urgently refers the woman to the backup obstetrician or
hospital at any time if there are signs of complications.
DURING DELIVERY
Preparation
1. Checks temperature in the DR area to be 25-28 degrees
centigrade and eliminates air draft.
2. Ensures a safe delivery by having:
- a clean surface for delivery that is se-upright
- all delivery equipment including newborn resuscitation
equipment
available and functional
- high level disinfected (HLD) instruments to cut cord.
- a pail with decontamination solution.
3. Arrange materials/supplies in a linear sequence.
4. Arrange in sequence another set of materials for
newborn care and supplies for after the first breastfeed.
5. Prepare a clean newborn resuscitation area on a firm
and flat surface.
6. Helps the woman unto the delivery table and ensures
that she is comfortable in the semi-upright position.
7. Drapes the woman properly to ensure privacy by proper
draping (e.g. leggings)
8. Cleans the perineum with antiseptic solution.
9. Washes hands and puts on 2 pairs of sterile gloves (if
without assistant)
10. Drapes the woman’s abdomen with clean cloth which
will be used to dry the baby.
During Delivery
11. Allows the woman to push as she wishes with clean
cloth which will be used to dry the baby.
12. Applies perineal support and controlled delivery of the
fetal head by instructing the woman not to bear down.
13.Wipes baby’s face to remove mucus and membranes, if
any.
14. Delivers the baby safety.
15. Calls out time of birth and sex of the baby.
First 30 seconds
16. Place the baby on mother’s abdomen in prone position
17. Thoroughly dries the baby for at least 30 seconds,
starting from the face and head, going down to the trunk
and extremities while performing a quick check for
breathing- just spread the vernix
1-3 minutes
18. Discards the wet cloth to start skin-to-skin contact and
wraps the baby with another clean, dry cloth and blanket.
Covers head of baby with a bonnet.
19. Palpates the abdomen to rule out another baby
(multiple pregnancy).
20. If assured of no other baby, correctly injects 10 IU of
oxytocin intramuscularly.
21. Remove, decontaminate and discard the first set of
gloves after positioning the baby for cord clamping.
22. Palpates the umbilical cord to check pulsations. After
pulsation stops, clamps and cuts the cord as follows:
- clamps cord with sterile plastic clamp at 2 cm and
places the instrument clamp 5cm from the base of
the umbilical cord
-Cuts the cord between clamps close to the first clamp
near the baby’s abdomen.
23. Waits for strong uterine contractions then apply
controlled cord counter traction on the uterus until the
placenta is delivered.
24. Checks the membranes and placenta to ensure that
the membranes and cotyledons are complete.
25. Immediately massage the uterus to keep it contracted.
26. Inspects the lower vagina and perineum for lacerations/
tears. Repairs it as needed.
27. Re-checks baby’s breathing, warmth and color and that
the mother’s uterus is contracted.
28. Advises mother to maintain skin-to-skin contact with
the baby prone on her chest with head turned to one side.
29. Checks that the uterus is contracted and massage it
through the abdomen when relaxed.
30. Cleans the vagina, perineum, and the inner thighs with
betadine.
31. Decontaminate and dispose the placenta in a leak-
proof container or plastic bag.
32. Decontaminates (soak in 0.5% chlorine solution) the
instruments for at least 10 minutes before cleaning and the
gloves before disposal.
15-90 minutes
1. Keeps mother and baby together and covers them with
blanket for warmth
2. Encourages the woman to breastfeed when the baby
shows cues that it is ready to feed (rooting reflex)
3. After a full breastfeed, administers eye prophylaxis (i.e.
Erythromycin ophthalmic ointment) and Vitamin K,
Hepatitis B and BCG injections to the infant.
4. Advises mother to:
- breastfeed on demand
- delay bathing the baby (not less than 6 hours after
birth)
Documentation
5. Records details of birth, including:
- Date and time of delivery and sex of the baby
- Baby’s length and birth weight
- Condition of the perineum and if any suturing was done
- Estimated blood loss
- Any changes from normal and referral
Evaluation
6. Performs a complete examination of the baby within the
first two hours of life and informs the mother of results.
7. Refers newborn for further care, if necessary, based on
examination of the baby.
8. Monitors mother and baby every 30 minutes for at least
2 hours or until stable
9. Encourages woman to continue taking liquids and eating
light foods as she desires.
10. Complete all records.
