Goal :To detect the presence and type of diplopia using red filter test

About the instrument:

Red filter scatters red light ,while light strikes upon it .


Prerequisite/Requirement:

Red filter,trail frame


Requires attentive and verbally adapt patient
Some patients have difficulty estimating spacing intervals,especially when the eyes are misaligned
in the two planes. The red filter test using the maddox rod test improve the reliability under these
circumstances.

Purpose of the test:

To detect the presence and type of diplopia of a patient


It can also be used to detect suppression, determine retinal correspondence, and to confirm the type
of diplopia a patient has (crossed or uncrossed).

Indications of the test:

When we have to determine weather the subject is having supression or ARC


To examine the visual feild of a patient having heterophoria.
Clinically, it is most useful to use this test on adults who complain of diplopia who seem orthophoric
on cover testing.
It is also a good way to fit adults with prisms.
Testing children with the red filter can be difficult

Principle of the test :

The red glass test relies on patients ability to report location of two different coloured lights.

Procedure of the test :

1. Make the patient seated comfortably and explain the procedure to the patient
2. The patient it is requested to see straight ahead at the torch light.
3. By convention red filter is placed on the right eye ( dominant eye )
4. Now, generally practitioner has to ask the questions :

How many lights they could observe ?

What is the colour of the light being observed by them ?

Precautions:

1. Make sure red filter is placed on dominant eye.

2. Make sure Patient target is not obstructed.

Interpretation :

If the eyes are aligned the patient will report one tingled light; No diploia.
If the eyes are out of alignment, the patient describes one white and one red light - Confirms
diplopia.
In esotropia the images appear in homonymous (uncrossed) diplopia, with the red light to the right
of the white one when the red filter is in front of the right eye.
In exotropia the images appear in heteronymous (crossed) diplopia, with the red light to the left of
the white light when the red filter is in front of the right eye. This represents NRC.
The patient may report that only one pinkish light in the position of the white fixation light is seen
i.e. the red and white images appear to be superimposed. This is clearly an abnormal response in
presence of heterophoria. This is termed Harmonious ARC.
The patient may report that two lights in uncrossed or crossed diplopia are seen, depending upon
the direction of deviation but the measured distance between the double images proves to be
smaller than expected from the magnitude of deviation. This represents unharmonious ARC.
Suppression is said to occur when the patient reports only a single light (usually the white light)
but occasionally red depending upon the density of the red filter and the degree of the dominance of
the fellow eye.
Patient with alternate fixation will see red and white lights alternatively.
