1. Kindly Carry this Receipt and Registration Certificate (RC) of the vehicle on your visit to the affixation center during
the scheduled appointment date / time.
2. Please ensure that the vehicle brought for affixation of HSRP matches with the HSRP order booking details. In case of
any mismatch, the order will stand cancelled and the entire booking amount shall stand forfeited.
3. Appointment can be re scheduled 3 times within a period of 90 days from the first appointment date. Failure to affix
HSRP within this time period will result in order cancellation and forfeited of the booking amount.
4. No additional charges are to be paid at the authorised affixation center towards HSRP affixation.
5. HSRP has to be affixed on the motor vehicle and will not be handed over to the citizen under any circumstances.

7. Warranty for 5 years: Applicable as per Rule 50 of CMVR 1989.
8. It is unlawful for any person to duplicate, alter, change, deface, destroy, mutilate, remove, teardown or takeaway the
'THIRD REGISTRATION PLATE STICKER' (where ever applicable) except if instructed by Authority of law or after
receiving all appropriate permission of the Regional Transport Authority of the state. Please do not wipe sticker or HSRP
with harsh abrasive materials, detergents, acid, alkane solutions, chemicals, solvent and any other removers, etc., as it may
disfigured.
