Dear Parent/Guardian,

​ Our school prioritizes and is committed to ensuring the well-being of all students. As part
of this commitment, we may conduct observations of students to better understand their
psychological, emotional, and behavioural needs, which will be instrumental for them to reach
the milestones expected of their age. As such, this letter stands as a formal request to seek
permission to observe your child for an indefinite period of time with the help of psychology
students from Cavite State University - Trece Martires City. The purpose of the said observation
are as follows:

●​ Identify potential challenges related to learning, attention, emotional well-being, and


psychological adaptation of the students.
●​ Assess and support the student based on their mental age and unique psychological
needs.
●​ Tailor interventions and strategies that can help them with their case and that which aim
to enhance their mental and emotional development.

Rest assured that this observation will follow the corresponding ethical rules.

Confidentiality
​ All findings related to and pertaining to this case will only be shared among the students,
the supervising guidance teacher, and the psychology students directly involved with the
observation. No disclosure of information shall happen without your consent, and the
information shall be kept in a secured database.

Voluntary Participation
​ The consentee has the right to grant or decline permission, as they have full supervision
in regards to this, as the representative of the students. And if at any point they wish to withdraw
their child, they are free to do so.
