Foreword I
When I was first approached by Dr. Hengli Tang about writing the foreword to this book,
the second edition of Hepatitis C Virus, I was a little puzzled: Book forewords are penned
by persons far more famous than I am. Surely there were better people to write this, with
a handful of the true visionaries in the field immediately coming to my mind. However,
after some thought and self-reflection, I realized that I might just be the right person for
the job.
I remember the publication of the first edition of Hepatitis C Virus in 1998, the year
I began reading about hepatitis C virus with great interest. I purchased a copy of the first
edition from my meager graduate student stipend and read it from cover to cover, along
with a variety of hepatitis C publications, to try to get a feel if this hepatitis C virus field
was something I might want to work on in the future. At the time, the first edition was
among the most comprehensive collection of hepatitis C protocols and methods available
in one volume. I was amazed at the progress that had been made in hepatitis C research,
and even more amazed as I learned more about the experimental challenges facing the
field at that time. Here was a system with only a single infectious molecular clone, no cell
culture replication system, no infectious virus culture system, no small animal model, and
only very limited structural information on the viral NS3 protein. Despite these significant
limitations, a considerable amount of information had been gathered on this virus using
patient data, bioinformatics, surrogate expression systems, the chimpanzee model, and a
vast array of in vitro methods. Even at that time, it was clear to me (and, I am sure, to many
others) that HCV was an unusual and fascinating virus. The previous edition of this book
helped me understand the major limitations facing the field, and highlighted the lengths
and efforts required to make progress on what was then one of the most challenging areas
of virology . . . both of which were invaluable in focusing my later research directions. I
joined the hepatitis C field in early 2000, at what might have been the perfect time, with
cell culture replicon systems making the study of RNA replication tractable, and I have
watched the field explode with amazing discoveries since that time. After this little bit
of retrospection, I thought I might actually be a good person to put the second edition
of Hepatitis C Virus in perspective. I have been actively working at the bench for the
last eight years, witnessing many of the new discoveries and technologies outlined in this
second edition. I have been lucky enough to have been involved in a few of these projects,
and observed many others at conferences and in the scientific literature with great respect
for the skill and insight of my colleagues. At every turn, hepatitis C virus and the dedicated
people who study it continue to amaze me.
So, here we are with a brand new second edition of Hepatitis C Virus, 10 years after the
publication of the first edition. It is clearly time for a new edition of this book, as so much
of hepatitis C research has changed in the past decade. I have already mentioned some
of the shortcomings of hepatitis C research ca. 1998, mostly focusing on the significant
issue of studying a virus that was clearly infectious in humans and chimpanzees, yet that
did not replicate or produce infectious virions in the cell culture environment. Perhaps the
biggest breakthrough in hepatitis C virus research in the last decade is the development of

efficient cell culture replicon–based replication systems, and more recently, the develop-
ment of a cell culture infectious virus system. These two developments, separated by more
than half a decade, have opened hepatitis C research to many labs and allowed for detailed
genetic and functional analysis of the hepatitis C life cycle. The replicon has evolved to
an efficient system allowing for forward and reverse genetic screens, and the information
garnered from these types of experiments has fueled incredible discoveries. This volume
includes chapters on the use of replicons, identification of revertant replicons by RT-PCR
sequencing, and inhibition of replication by siRNAs. There are also chapters on apply-
ing biochemistry to the replicon system, including the visualization and purification of
the active RNA replication complex from cells, the analysis of NS5A phosphorylation in
replication experiments, and the development of in vitro replication systems. Mathemat-
ical modeling of the kinetics of RNA replication is also described. The replicon has also
served as a valuable screening tool for small molecule anti-viral development. At the time
the last edition of this book was published, interferon-ribavirin was state of the art, with
pegylated interferon and ribavirin therapy arriving three years later. These therapies have
increase sustained virologic responses in patients dramatically, but more work is clearly
needed. This volume describes the development of drug screening replicon tools used
and in use to identify more effective specific small molecule anti-virals. The development
of anti-viral resistance upon exposure of replicons to these new anti-virals, as described
herein, has allowed rapid identification of potential compound binding sites and aided in
drug development, as well as highlighted the need for more anti-virals. The virus system
has also allowed for significant advancements in our understanding of hepatitis C biology.
This book features chapters on the use of the infectious virus system, measuring virus
infectivity, generating chimeric viruses, and adaptation of virus to cell culture to increase
the utility of the system. The virus system has led to interesting discoveries in the area of
the functions of p7 and NS2 to virus production, and more recently, the roles of NS5A
and NS3 in this process. It also features sections on the application of the infectious cell
culture virus system and the pseudoparticle systems to the poorly understood areas of virus
receptors and entry. The vast majority of replicon and virus research has been conducted
on a variety of adapted subclones of a human hepatoma cell line. Although these lines
have been incredibly useful in characterizing many aspects of hepatitis C biology, they are
clearly quite different than authentic human hepatocytes. A chapter in this book describes
recent advances in the infection of isolated primary hepatocytes in cell culture allowing for
a more direct view of the virus lifecycle in the presumed authentic target cell.
The structural biology of hepatitis C has seen great advances in the last decade, pro-
gressing from isolated structures of the NS3 protease and helicase domain at the time
the first edition was published to where we are today with crystal structures of NS2, the
complete NS3/NS4A complex, a portion of NS5A, and NS5B. It is worth noting that
structures of NS3 and NS5B with ligands ranging from nucleic acids to small molecule
inhibitors have greatly enhanced our knowledge of these proteins. NMR structures have
also been determined for the membrane anchors of NS5A and NS5B and provided a view
of a portion of the core protein. Cryo-electron microscopy and structural biology of the
viral RNA in the IRES region has greatly enhanced our understanding of how HCV RNA
functionally engages the ribosome. The text in this edition focuses on the preparation of
NS3, NS5A, and NS5B for structural studies, as well as some exciting new work on the
structural characterization of the p7 protein.
Immunology has seen great progress as well. The investigation of the complex inter-
play between the virus and the host immune response has produced some amazing and

unexpected results. It is clear that hepatitis C plays an active role in immune evasion by
a variety of methods including mutation-based evasion of immune recognition, cleavage
of a number of innate immune response pathway adaptor proteins, manipulation of host
cell signaling, modulation of cytokine levels, masking of virions with lipids, and many
other fascinating mechanisms that remain to be discovered. It is increasingly clear that the
maintenance of long-term persistent infections requires a complex, multi-faceted approach
by the virus . . . one we are only beginning to decipher. This text features chapters on
the interplay of hepatitis C with IRF3 signaling and the characterization of the immune
response to infection, as well as a chapter dealing with the exciting idea of using of mass
spectrometry-based analysis of hypervariable regions of the hepatitis C proteins.
Another area that has seen significant development in the past decade has been the
technology used to detect and quantitate HCV RNA and proteins in both clinical and
research-based environments. The sensitivity and reproducibility of these assays have made
great strides in the past decade, allowing very accurate measures of viral load in a clinical
setting. It has become commonplace to monitor RNA and protein levels in the patient,
genotype the patient’s infection, and apply this information to optimize therapy. A chapter
in this text compares these quantitation methods. Our understanding of hepatitis C geno-
type distribution, in light of the large numbers of HCV isolates sequenced to date, has
continued to evolve, and the current nomenclature and classifications are also presented.
Instead of trying to be exhaustive and provide methods for all the work that has been
performed since the publication of the first edition, which would be impossible, this book
focuses on significant methods that can be applied to a wide range of projects. Looking
back at the last decade, the discovery and progress toward understanding hepatitis C has
occurred at an incredible pace. This is largely due to the incredibly bright, dedicated, and
creative people working on this important pathogen. I can hardly wait to see what the

Our understanding of hepatitis C virus (HCV) has been largely restricted by the availability
of tools to dissect the virus and its relationship with the host. Thus, new aspects of HCV
biology have been revealed through a number of technological breakthroughs and clever
experimental designs. As we close in on the third decade of HCV research, we can be sure
that these advances have carried us a long way. The study of HCV began with its identifi-
cation through cDNA cloning of the viral genome. From there, the viral genes and their
gene products were studied, the first chimp-infectious cDNA clones were constructed,
and subgenomic replicons were built. The importance of this last point cannot be under-
stated, as it ushered in a new era of studying viral replication, aided in the discovery of
HCV-specific inhibitors, and most recently, culminated in the development of systems to
grow infectious HCV in cell culture. At the host-pathogen interface, we have identified
some of the cellular machinery necessary for infection and genome replication, gained a
basic understanding of innate and acquired immune responses to the virus, and learned
of some tricks the virus uses to counteract these defenses. Of no less importance, we have
developed small animal models of HCV infection and improved our understanding of
HCV dynamics in vivo.
In many ways, the book you hold in your hands reflects the progress of discovery in
HCV research. In the first part we learn how to detect the virus (Chapters 1–3), charac-
terize the viral genome (Chapters 4–6), and study its gene products (Chapters 7–10, 16).
The introduction of HCV replicons (Chapter 11) provides us with the basic tools to exam-
ine functional HCV RNA replication complexes (Chapters 12–15) and develop inhibitors
of their function (Chapters 17–19). Expanding our view of the virus life cycle, we learn
how to study HCV entry by using surrogate systems (Chapters 20–22) and to tame the
virus in cell culture (Chapters 23–27). We next turn to understanding the host-pathogen
interaction, with protocols to study HCV infection in culture primary hepatocytes
(Chapter 28) and mice bearing human liver grafts (Chapter 29), as well as a series of
methods to study the adaptive immune response to the virus (Chapters 30–32). Tying all
of this together, Dahari and colleagues demonstrate how mathematical modeling can be
used to provide the big picture of HCV dynamics in vivo.
Still, our understanding of HCV remains incomplete, and there is much work to be
done. This book is not so much a chronicle of the past as it is a guide to the future. The
protocols here represent the state of the art in HCV research, and importantly, were all
written by researchers directly involved in developing these techniques.
Congratulations to Hengli for editing this volume, and a sincere thanks to my fellow
contributors. To readers of the book, I say “Good luck,” but must confess a certain kind
of envy ... I only wish I had been able to find such a great book of protocols years ago!

Preface to the Second Edition
The first edition of this book, published in 1998, presented a large collection of author-
itative protocols from the first decade of HCV research, outlining clinical diagnostics,
genotyping, and molecular biology of the virus. One of the largest obstacles to produc-
tive research on the HCV life cycle had been the lack of a robust cell-culturing system
capable of HCV replication and infection. We now possess such systems, and as a result,
the past decade has witnessed an explosion of innovative research directed at understand-
ing HCV biology and, more specifically, RNA replication in cell-culture replicon models.
More recent advances in the production of infectious particles and infection of cultured
hepatoma cells as well as chimpanzees will surely open a new chapter of studies examin-
ing the full life cycle of the virus by means of the new infection model. This advance is
especially important for traditional virologists and immunologists, as they are now able to
study viral assembly, entry, and pathogenesis as well as the innate and adapted immunity
of the host. In other words, we have finally reached a point where virtually all the impor-
tant questions regarding this human pathogen and its interaction with the host can be
addressed in a laboratory setting.
This new edition of Hepatitis C strives to reflect this major thrust in the basic research
of HCV and to provide a compilation of cutting-edge research techniques that are cur-
rently used by HCV labs worldwide to study HCV infection and replication in vitro.
In addition, in keeping with the style of the first edition, the book opens with updated
chapters discussing important methods of clinical detection and quantification of HCV
infection. These are followed by several chapters detailing current systems for classifying
HCV genotypes, numbering HCV sequences, and determining quasispecies. Finally, the
volume concludes with protocols for the characterization of immune responses to HCV
and a unique chapter describing mathematical modeling of HCV RNA kinetics. We hope
that both basic scientists and clinical researchers will find this book useful as a reference
guide to lab research using HCV as a model.
