Gonioscopy is a clinical technique used to examine the anterior chamber angle. It remains the gold standard for anterior chamber angle assessment but has limitations including subjectivity and interobserver variability. Gonioscopy can be performed directly using specialized contact lenses or indirectly using lenses with mirrors. Angle structures are graded using systems like Shaffer or Scheie to classify angle width and visibility. Gonioscopy is used diagnostically to detect angle closure and guide treatment with laser peripheral iridotomy as well as surgically during procedures like goniotomy. Newer objective techniques like ultrasound biomicroscopy and anterior segment OCT are developing but have not replaced gonioscopy.
