proudly presents:
The 2nd Annual Interprofessional Evidence-Based Conference
Transitions of Care: Show us the Evidence!
Interprofessional practice is a requirement for 21st century
healthcare. No longer can healthcare organizations, clinicians or
consumers afford for disciplines to be siloed in their thinking,
their practice, or their decision-making. We are particularly excited
about our 2nd Annual EBP Conference as we are able to offer
participants wonderful opportunities to catch up with the latest
evidence on interprofessional practice and team learning along
with how clinicians work together to provide seamless transitions
of care to improve patient outcomes. With growing interest across
the state and nation in transforming healthcare through
interprofessional evidence-based practice and education,
we invite you to join us for this exciting conference!
