the information received, we tailor the program to the enrollee’s specific needs.
 Personal Training and Exercise Programs - Available on the Plan Effective Date
 Our trainers design training and exercise programs tailored to the needs and goals of the employee and monitor
their progress along the way.
 Nutritionists - Available on the Plan Effective Date
 Our nutritionists are experts in the field of food and nutrition. They advise employees on what and how to eat in

 Biometric Screening - Available 3 Months (90 days) After Plan Effective Date
 Biometric screening can identify risk factors including diabetic and cardiovascular disease up to ten years
before full onset. Screening data and analytics allow our wellness coaches to provide effective tactics and
monitor effectiveness.
 Fitbit- Subject to participation in Biometric Screening
 Employees receive a $50 credit towards a Fitbit
 DNA Screening - Available 9 Months After Plan Effective Date
 Knowing there is a mutation in one of the 19 genes analyzed — including BRCA1 and BRCA2 — enables
employees to work with your healthcare provider to create a personalized plan designed to prevent or
detect breast and ovarian cancer at an earlier or more treatable stage.
 Mammograms - Available 1 Year After Plan Effective Date
 Regular mammograms can often help find breast cancer at an early stage, when treatment is most likely
to be successful.
 Prostate - Available 1 Year After Plan Effective Date
 Prostate cancer screening can help identify cancer early on, when treatment is most effective. And a
normal PSA test, combined with a digital rectal exam, can help reassure an employee that it’s unlikely he
