This document summarizes a novel cardiac pacing paradigm called CRT+CP (cardiac resynchronization therapy combined with coupled pacing) for patients with atrial fibrillation (AF) and heart failure (HF). CRT+CP applies CRT to synchronize ventricular contractions followed by a third stimulation to control heart rate during AF via coupled pacing. Animal studies found CRT+CP increased left ventricular function and reversed HF remodeling compared to other pacing therapies. Simulations suggested measuring differential pressure in the great cardiac vein could optimize CRT+CP by monitoring cardiac flow.
