Nursing Process
o The nursing process is a systematic, rational
method of planning and providing
individualized nursing care.
o A systematic method of providing care to
clients
Nursing Process
o The nursing process is a critical thinking
process that professional nurses use to
apply the best available evidence to are
giving and promoting human functions and
responses to health and illness
(American Nurses Association, 2010)
Purposes of Nursing Process
o To identify a client’s health status and actual or
potential health care problems or needs.
o To establish plans to meet the identified needs.
o It provides a framework in which to practice nursing.
o Provide care for clients that is individualized, holistic,
effective, and efficient.
o Protects nurses against potential legal problems
Benefits of Nursing Process
o Provides an orderly & systematic method
for planning & providing care
o Enhances nursing efficiency by
standardizing nursing practice
o Facilitates documentation of care
Benefits of Nursing Process
o Provides a unity of language for the nursing
profession
o Stresses the independent function of nurses
o Increases care quality through the use of
careful actions
Characteristics of Nursing
Process
o Cyclic
o Dynamic nature
o Client centeredness
o Focus on problem solving and decision
making
o Interpersonal and collaborative style
o Universal applicability
o Use of critical thinking and clinical reasoning.
Steps / Phases/Components of
Nursing Process
o Current Scope and Standards of Nursing
Practice include six phases of the nursing
process:
1. Assessment,
2. Diagnosis,
3. Outcomes identification,
4. Planning,
5. Implementation, and
6. Evaluation
Most Uses ADPIE Five Steps /
Phases/ Components of Nursing
Process
1. Assessment
2. Diagnosis
3. Planning and Outcomes identification
Implementation
4. Evaluation
Most Uses Five Steps / Phases
of Nursing Process
Requirements for Effective Use of
the Nursing Process
o Nursing knowledge is required from many
fields, including:
• Natural science, Social science, Behavioral
science, Arts and humanities, and Nursing
science.
Requirements for Effective Use of
the Nursing Process
• Referral and collaboration among nurses
and other health care professionals
• Critical thinking and decision making
• Use of evidence-based nursing
interventions.
Requirements for Effective Use of
the Nursing Process
• Reinforce for application of the all steps of
Nursing Process.
• Adequate time
• Nursing human power and adequate salary
• Near to the ground workload
• Materials.
• Arrange Training for Nurse
11FUNCTIONAL HEALTH
PATTERNS
o Gordon’s (2002) human functional health
patterns model provides a systematic
framework for data collection that focuses
on 11 functional health patterns.
11FUNCTIONAL HEALTH
PATTERNS
o Health perception-health management
pattern.
o Nutritional-metabolic pattern
o Elimination pattern
o Activity-exercise pattern
o Sleep-rest pattern
o Cognitive-perceptual pattern
11FUNCTIONAL HEALTH
PATTERNS
 Self-perception-concept pattern
 Role-relationship pattern
 Sexuality-reproductive pattern
 Coping-stress tolerance pattern
 Value -Belief pattern
Health perception–Health
management pattern
Describes client’s perceived
pattern of health and well-being
and how health is managed
Nutritional-Metabolic Pattern
Describes pattern of food and
fluid consumption relative to
metabolic need and pattern
indicators of local nutrient supply
Elimination Pattern
Describes patterns of excretory
function
(bowel, bladder, and skin)
Activity-Exercise Pattern
Describes pattern of exercise,
activity, leisure, and recreation
Cognitive-Perceptual Pattern
Describes sensory-perceptual
and cognitive pattern
Sleep-Rest Pattern
Describes pattern of sleep, rest,
and relaxation
Self Perception–Self Concept
Pattern
Describes self-concept pattern
and perceptions of self
Role-Relationship Pattern
Describes pattern of role
engagements and relationships
Sexuality-Reproductive Pattern
Describes patterns of satisfaction
or dissatisfaction with sexuality;
describes reproductive patterns
Coping Stress Tolerance Pattern
Describes coping pattern and its
effectiveness in stress tolerance
Value-Belief Pattern
Describes goals and value and
belief patterns that underlie
decision making
Conclusion
o The nursing process is not lined but involves
overlapping steps that build on each other
(Assessment, Diagnosis, Outcome Identification and
Planning, Implementation, Evaluation)
o Effective use of the nursing process required
knowledge, collaboration among nurses , Critical
thinking, decision making, application of the all
steps, nursing human power and Materials.
o Gordon’s human functional health patterns model
provides a systematic framework for data collection
that focuses on 11 functional health pattern
