  Introduction: MRD is abasic tool for any Hospital administration to furnish information about any patient’s admission, discharge, birth, death, medico-legal certificate, clinical information, operation performed, etc and release of hospital’s vital statistical information, including parliament question/query and RTI query reply.

  Aim: To provide informationto Hospital administration, Govt., Police, Public, Judiciary, NGO, National/International Agencies about Patient care and for smooth functioning and future planning of Hospital Industry.

  Methodology: MRD collects allthe information from different wards, Casualty, CRC ,ICU, OTs, OPDs, and compiles these case sheets, prepares vital statistical information and then presents it, in desired format to Hospital administration or agency. Besides, MRD-TC have issued MR documents to judiciary regularly.

  Results: The below mentioneddata had been collected by MRS staff during calendar year 2014 from JPNATC- AIIMS New Delhi.

  Results Contd... The totalnumber of patients attended at casualty were 62,343 having daily attendance of 171pts out of these 28285 pts were reported Medico legal cases (55%MLC cases) 34058 non MLC cases (45% NMLC). 5822 (9.3%) cases were needed admission during 2014. In which 5505 were reported major operations cases and 317 cases were minor cases.

  Results Contd... Surgery specialtyhad reported highest operations 2178, Ortho 2075, Neurosurgery 1471 cases of operation. 53380 days of care were rendered to patients, 840 Deaths had been reported, out of these 282 deaths were reported Brought Death at casualty. In concise, after thoroughly evaluating and using statistical tools & techniques ,the hospital performance indicators are shown as GDR 11%, NDR 06%, BOR 83%, ALOS 10days and BTOR 30pt/bed.

Besides, thesehospital statistical indicators, 2005 court summons were attended by various Doctors and MRS staff. 4075 Medical record files were issued to various medical researchers for study and reference.840 death cases were reported to NDMC (B&D Registrar) on-line as well as off-line. 02 cases of foreign nationals admitted, and were reported to FRRO by MRS. Near about 10,000 Medical Record paper documents of patients with attest have been submitted to various Law Courts during 2014. All above inputs and more, were provided to various authorities and Hospital administration in the interest of speedy and smooth functioning of patient care.

  Conclusion: MRD plays veryimportant role in delivering patient care treatment effectively, as we all know “Patient forgets but record remembers”
