The second heart sound (S2) is produced by the closure of the aortic and pulmonary valves. Normally, the pulmonary valve closes after the aortic valve, producing the physiological split of S2 that widens during inspiration. A wide split S2 that persists or widens during inspiration can be caused by delayed pulmonary closure or early aortic closure. Reversed split of S2, where P2 occurs before A2, widens during expiration and narrows during inspiration, and is caused by delayed aortic closure or early pulmonary closure. The expiratory split of S2 can be wide, fixed, or reversed.
