A 9-year-old boy was referred to an ENT specialist due to a non-resolving cough and wheeze that had lasted 45 days. A physical exam found wheezy chest bilaterally. A chest X-ray was normal. During rigid bronchoscopy under general anesthesia, a sunflower seed shell was found lodged in the middle lobe bronchus, causing severe mucosal congestion. The foreign body was removed using forceps and the bronchus was irrigated with saline. The boy's condition improved after the procedure and he was discharged the next day in good health. Despite a negative history and normal X-ray, unresolved cough in children should raise suspicion of a foreign body aspiration,
