PILOT PROGRAMME… Launched on 4 th  jan 2008 In 7 states with 1district each from   Assam  Punjab Rajasthan Karnataka Tamilnadu Kerala Andhrapradhesh Financial outlay for pilot phase is 5 crores

STEP 1: LEARN ABOUT THE DIABETES   Diabetes means that the blood glucose is too high.  These are the main types of diabetes Type 1 diabetes  Type 2 diabetes  Gestational diabetes
STEP 2: KNOW DIABETES THE ABCS The ABCs of Diabetes Control  A = A1C B = Blood pressure C = Cholesterol
STEP 3: MANAGE THE DIABETES
DIABETES FOOD PYRAMID Eat healthy foods such as   fruits and vegetables  fish lean meats chicken or turkey without the skin  dry peas or beans  whole grains low-fat or skim milk  cheese
Eat foods with more fiber such as  whole-grain cereals  breads  Rice or pasta Diabetes Of Food Pyramid  ….
Diabetes Of Food Pyramid  …. Keep fish and lean meat and poultry portions to about 3 ounces Eat foods that have less fat and salt
DIABETES MEAL PLAN  …. Get 30 to 60 minutes of physical activity  Stay at a healthy weight  Learn to cope with stress Stop smoking  Take medicines even when feel good
STEP 4: GET ROUTINE CARE   Seeing health care team at least twice a year to find and treat any problems early   At each visit be sure you have a: blood pressure check Review of your self-care plan Foot Check Weight Check
Get routine care……… Two times each year have an: A1C test – it may be checked more often if it is over 7   Once each year be sure you have a: Cholesterol test  Triglyceride test  Complete foot exam
Get routine care… Dental exam to check teeth and gums  Dilated eye exam to check for eye problems  Urine and a blood test to check for kidney problems
