🔜 Happening soon at the Home Care 100 Conference in Phoenix! This morning’s panel takes on a timely topic: value-based care contracting in Medicare Advantage, through the lens of home-based care. Grady Murphy, Market President at Pearl Health, will join the discussion to share perspective shaped by real-world experience navigating MA contracting—focused on how contract structure, incentives, and accountability show up in practice, not just on paper. While the conference is centered on ACOs, the conversation reflects a broader reality: as risk continues to shift, MA contracting decisions are becoming more operational, more consequential, and more time-sensitive. We're looking forward to a thoughtful discussion with fellow leaders shaping what’s next in value-based care. #HomeCare100 #ValueBasedCare #MedicareAdvantage #HealthcareLeadership

🚨 Don’t trust LEAD underwriting - yet. With the LEAD model announced, confident projections are already circulating. But underwriting is only as good as knowing the model’s underlying structure—and much of that is still TBD. In our latest article, our COO, Gabe Drapos, and Danny W. (Senior Manager, Data Science) explain what actually determines underwriting reliability and when the numbers are worth trusting. Post-discharge care is one of the most preventable failure points in value-based care yet one of the hardest to manage at scale. As organizations take on more downside risk, identifying who needs follow-up and when has become a real operational challenge. In this new blog from Pearl Health, Yuxin C. and Patrick Mauro break down how advanced AI can materially improve post-discharge decision-making while covering: • Why diagnosis codes alone miss meaningful post-discharge risk • How patient similarity and clinical embeddings surface earlier intervention opportunities • What population-level risk visualization enables for care teams managing limited resources The takeaway? Better post-discharge care isn't about more alerts. It's about clearer prioritization and earlier action. CMS has announced that the new 𝗟𝗼𝗻𝗴-𝘁𝗲𝗿𝗺 𝗘𝗻𝗵𝗮𝗻𝗰𝗲𝗱 𝗔𝗖𝗢 𝗗𝗲𝘀𝗶𝗴𝗻 (𝗟𝗘𝗔𝗗) 𝗠𝗼𝗱𝗲𝗹 will replace 𝗔𝗖𝗢 𝗥𝗘𝗔𝗖𝗛 starting January 1, 2027 and run through 2036. It’s a major signal of CMS’s long-term commitment to accountable care, with a renewed focus on stability, prospective payments, and support for rural, high-needs, and dual-eligible populations. In our latest blog post, Pearl Health’s VP of ACO Operations & Strategy, Gillian Christie, breaks down what we know so far and what provider organizations should be paying attention to as details continue to emerge. Key questions we’re watching: • how benchmarks and risk adjustment will be structured • what the incentives look like for independent providers • how specialist integration through CMS Administered Risk Arrangements may work The Request for Applications is expected in March 2026, but now is the time to understand the framework and start planning for what’s next. 👉 Read the full breakdown here: https://lnkd.in/eQ8XvnKT #valuebasedcare #healthpolicy #CMS #ACO #healthcareinnovation #LEAD

Healthcare leaders don’t get the luxury of stability. Policy shifts, payment reform, margin pressure, and workforce strain are constants, not exceptions. The organizations that outperform aren’t reacting faster. They’re designed differently. In our latest piece by Pearl Health Chief of Staff Ciara Walsh, we outline what separates resilient healthcare organizations from those stuck in perpetual firefighting, including how leading teams: • translate strategy into clear, executable priorities • design operating models that absorb change without burning out teams • hold complexity at the leadership level so frontline teams can stay focused • build organizational muscle for adaptation, not just short-term optimization. The takeaway is simple but often overlooked. Sustainable performance in healthcare is less about heroic leadership and more about how teams are structured, supported, and empowered to execute amid uncertainty. 🔗 Read the full piece: https://lnkd.in/eRMyq2Y3 #HealthcareLeadership #ValueBasedCare #HealthcareOperations #ExecutiveLeadership #OrganizationalDesign Michael Kopko Gabe Drapos Dennis Weaver, MD, MBA Dennis Hillen Steven Duque Jonathan E. 𝗛𝗲𝗮𝗹𝘁𝗵𝗰𝗮𝗿𝗲 𝘀𝗽𝗲𝗻𝗱𝗶𝗻𝗴 𝗶𝘀 𝗿𝗮𝗱𝗶𝗰𝗮𝗹𝗹𝘆 𝘀𝗸𝗲𝘄𝗲𝗱. 📈 The top 5% of patients account for nearly half of all costs. The top 1% alone drives more than twenty percent of total healthcare spending. The real insight is not the imbalance. It is how predictable these costs are. In our latest post, Pearl Health CEO Michael Kopko breaks down how precision AI can identify risk earlier, guide next best actions, and reduce avoidable and costly admissions before they happen. This is how you bend the cost curve 𝑤𝑖𝑡ℎ𝑜𝑢𝑡 𝑐𝑢𝑡𝑡𝑖𝑛𝑔 𝑐𝑎𝑟𝑒. 📉 👉 Read more: https://lnkd.in/ef6ubYit #HealthcareEconomics #ValueBasedCare #PrecisionAI #PopulationHealth #CareDelivery
