We're excited to announce our launch into New York State! New Yorkers across the state can now access effective, compassionate treatment for opioid and alcohol use disorders and kratom dependence, completely online. According to the New York State Office of Addiction Services and Supports, the Empire State had an estimated 4,449 deaths from drug overdose in 2024, 77% of which involved an opioid. This is a devastating loss, but the impact of opioid use disorder in the state extends far beyond the death toll statistics. Addiction affects hundreds of thousands in the state, as well as their families, businesses, and communities. The New York State Department of Health also reports that nearly 1 in 6 adults in New York State (16.4%) reported excessive alcohol use in the form of either binge or heavy drinking. Via Workit Health's app-based programs, residents from all corners of the state can access evidence-based care through their smartphones. Through our telehealth programs, licensed clinicians can prescribe medications like buprenorphine and naltrexone (as appropriate) to relieve cravings and support recovery. Workit members attend video appointments with licensed clinicians, behavioral health staff, and fellow members of Workit Health. They can also build connection through online recovery groups and members-only forums. Our providers also offer care for several commonly co-occurring conditions like anxiety, depression, insomnia, and hepatitis C, and prescribe PrEP for HIV. The furor about kratom and 7-OH has taken many clinicians off guard. In this clinical reflection from Workit Labs, the authors address the complexity of kratom use—its risks, the challenges of treatment, and reasons why individuals choose to use it. Kratom (Mitragyna speciosa) is derived from trees native to Southeast Asia. It behaves like a stimulant at low doses and like an opioid at higher doses. One of kratom's active alkaloid compounds, 7-hydroxymitragynine (colloquially called 7-OH), has a greater opioid-like effect and an increased risk for physiologic dependence. Clinicians need to be aware of kratom and 7-OH, as more patients may be employing them as self-managed solutions for pain, mental health issues, or other substance use disorders; may be using them recreationally; or may be dependent on them and struggling with the cycle of withdrawal. This overview is an excellent place to start for clinicians unfamiliar with #kratom and #7OH use, how kratom use disorder is commonly treated, and policies that may reduce the public health risks of these substances. She completed her internship and residency in Internal Medicine and a Fellowship in Sleep Medicine at the UNM HEALTH SCIENCES CENTER. Dr. Begay practices at a New Mexico Sleep Center and is an Assistant Professor in The University of New Mexico (UNM) Department of Pulmonology, Critical Care and Sleep Medicine. We love to see this clear demonstration that engaging in our program is improving our members' recovery and quality of life. Our members' overall recovery scores on the Substance Use Recovery Evaluator (SURE)—a validated assessment that captures 5 recovery domains—improved by approximately 10 points within the first three months of treatment. In the Drinking & Drug Use domain (which includes items such as cravings over the last week), the improvement is even more significant, with an average increase of 32 points from baseline to three months, indicating substantial improvement in substance-related symptoms. Our care team uses members' SURE scores to identify emerging risks; tailor interventions, resources, referrals, and follow-ups; and track progress and adjust care plans over time. Far from being a hindrance, that experience can give them the insights and resources to help others in recovery.
